中文摘要 | 因為術中出血較少,術後傷口較不浮腫疼痛,使用二氧化碳雷射來做乳房切除手術已逐漸普遍。1988年Becker使用雷射於小針美容病人的乳腺矽化物時見有白熱火花產生而不知其所以然。作者有幸做進一步的探討,此乃有關神奇火花之初步結果報告。 小針美容乃經皮注射矽化物(醫療用或工業用?)並添加某些油性物質(雜質?)而在人體內引起炎症纖維化以達到局部軟組織填充整形的效果(Sakurai之處方)。東方人尤其用在胸部和顏面的美容。由於高純度的矽化物具有穩定性及高耐熱性不生火花,故作者假設雷射火花和小針美容的矽化物雜質有關而做一連串的研究。 首先將市售常見的油性物質,諸如Olive oil,mineral oil,vegetable oil,petrolatum,paraffin,glycerin等樣品在實驗室直接以二氧化碳雷射照射,同時以silicone oil和人體脂肪腫瘤之脂肪組織做對照,結果都無預期的火花產生。接下來探討是否尚有其他成份添加物,而使用TLC,HNMR和IR等化學分析法,結果顯示其主成份為矽(非paraffin),而添加的"雜質"極其微量,目前無法測知。 此外,雷射照射條件改變時,在高能量(大於8W)才有白熱火花的現象,而低於5W則往往只見煙霧,沒有火花。推論此等雜質具有較低的熔點,一般的金屬和有機化合物屬之。 綜合上述研究結果顯示,雷射矽化物火花的光熱現象證明了另一個事實,那就是在小針美容注射的矽化物中是存在有"未知"的雜質混合物。或許往後研究能夠採用更精密的儀器,如HPLC-Mass可以分離追察每一可能的成份,再分別以雷射處理以找出何種成份會引起火花。若仍無定論,那麼或許火花的產生只是人體組織對於矽化物雜質的反應而形成未能解釋的物理化學甚至生物分子的存在現象呢。 |
英文摘要 | "Sakuraiformula" injectable silicone had long been performed in the Oriental people as augmentation mammaplasty and sometimes with the complications of induration, redness and/or pain over pectoral region. Subcutaneous mastectomy may be the only solvable choice for it. The laser silicone flash, a visible incandescent flash phenomenon associated with use of the carbon dioxide laser in vaporizing silicone deposits in a breast was noticed before without explanation. This study is possible a milestone trying to explore the relationship between "silicone flash" and "Sakurai formula". In laser studies, with high power (above 8W), it caused flash easily while lasering the cystic clear viscid content over themammary silicone granuloma specimen, but it caused only steam without flash with lower power (below 5W). The commercially available adulterants also got no flash phenomenon with 8W lasering. In chemical studies, the major component of extracted siliconoma solution was identified to be dimethylpolysiloxane fluid("pure" silicone oil) by thin layer chromatography (TLC), nuclear magnetic resonance (H-NMR), and infrared spectrophotometry (IR) respectively but the added impurity of Sakurai formula that caused laser silicone flash still cannot be identified by now. The further investigation with HPLC-Mass is recommended. |
