中文摘要 | 自1987年12月至1989年11月之兩年間,吾人共做了73例之體外骨髓培養及骨髓之詳細抹片檢查,其中16例為對照組,8例為慢性骨髓性增生疾病,16例為急性骨髓性白血病及33例之造血不良症候群(MDS)。百分之四十五之MDS病例有核間橋樑連接的現象(INB)(如圖1)、此種發現為其他病例所未見(除了極少數之急性骨髓性白血病外),但也見於50%之巨母芽球貧血之病人(11/22)。因此INB之存在為MDS之重要發現,但並不能據此診斷,且與MDS之亞型並無相關,且對生存期並無明顯的影響。 體外骨髓培養以觀察其細胞生長情形發覺MDS與急性骨髓性白血病頗為接近,而與其他兩組截然不同。當集落之數目越少時(<5),對其預後有不良影響之趨勢,但與□5之另一組比較則未達統計上之意義。吾人試圖把MDS分為INB(-)及集落生長<5作為一組與剩餘之另一組做比較,發覺前者急性白血病之轉換率高於後者(6/12 VS 2/19)而達統計上之意義,此點似可做為預後分析之參考。 |
英文摘要 | Thirty-three patients with FAB-classified primary myelodysplastic syndrome (MDS) were investigated for in vitro growth of colony-forming units for myeloid precursors (CFU-C) and microscopic searching for internuclear bridging (INF). We found that 45% of MDS and INB was neglected in the past, and those with very low CFU-C count had a tendency towards worse prognosis. Combining low colony count and the absence of INB, we found that they had a higher acute leukemia transformation rate and that the changes might be conductive to the prognostic evaluation in MDS. |
