中文摘要 | 在229個正常足月期之孕婦中,以放射定量法測定血清維生素B12值,然後加以分析。此些孕婦在懷孕期間均未服用任何補血劑。其平均年齡為27.70歲,而平均懷孕週數為39.51週。平均血色素濃度則為12.59gm%。所測出之血清維生素B12平均值為307.5pg/ml(226.9pmol/L)。此平均值比起同年齡正常婦女之血清B12平均值(557.5pg/ml或426.1pmol/l)低出很多。對孕婦血清維生素B12之降低,本文提供我們的意見。在本實驗中,有12.6%之正常孕婦(29/229),其血清B12值少於150pg/ml,然其中一人有貧血(Hb<11gm%)之現象。因此,正常中國孕婦雖然會有血清維生素B12降低之現象,但卻很少產生維生素B12缺乏性貧血症。多產、年齡、多孕等均與B12之降低無關。而血色素濃度與血清B12之濃度似乎在孕婦中亦無直接關係。總括而言,懷孕時之低血清B12濃度並不能代表體內之B12總量,故我們不能只以B12降低即診斷孕婦有維生素B12缺乏症。但另一方面,倘若血清B12為正常或高於正常,則可以較肯定地排除維生素B12缺乏症之可能。 |
英文摘要 | Serum cobalamin levels were determined by radioassay and the analysed in 229 normal full term pregnanat women. None of them had received any hematonic during their pregnancy. Their mean age was 27.70 years and the mean duration of pregnancy was 39.51 weeks. Mean hemoglobin concentration in these normal pregnant women was 12.59 gm%. Mean serum cobalamin was 307.5 pg/ml (226.9 pmol/1). It was much lower than the mean value (557.5 pg/ml, 426.1 pmol/l) of normal non-pregnant women at the same age. Our explanation of the fall of cobalamin is given. In this survey, 12.6% (29 out of the 229) of the normal pregnant woment had a cobalamin level less than cobalamin deficiency anemia is rarely seen although the serum cobalamin level is significantly lower during pregnancy. Multiparity, age and gravida number all played no role in the occurrence of cobalamin deficiency. No positive relationship appeared between serum cobalamin level and hemoglobin concentration in Chinese pregnant women. In conclusion, we found that low serum cobalamin level itself cannot represent the real cobalamin status in pregnanacy, and we cannot diagnose cobalamin deficiency in pregnant women based only on the low serum cobalamin level. On the other hand, if serum cobalamin is found to be normal or higher than normal, it is certain to rule out the possibility of cobalamin deficiency. |
