中文摘要 | 背景:急救是急診護理人員必備的重要技能之一,其過程包括需要與其他醫療團隊成員的合作中,正確操作急救技能、辨識心律變化,以及後續針對病人得正確的處置等。本專案施行地點為新成立的一般級急救責任醫院之地區醫院急診室,2020年10月1日至10月31日,以自擬「急診護理人員執行病人急救處置作業觀察表」觀察急診護理人員執行急救處置作業正確率僅62.1%。透過訪談後分析正確率偏低原因為:1.單位無針對新進護理人員適切的急救教育訓練課程;2.多數人員護理年資淺,面對急救事件實務經驗不足;3.工作常規缺乏急救團隊職責工作分配以致工作分配不明確。目的:提升急診護理人員執行急救處置正確率由原62.1%提升至91%。方法:針對急救團隊成員職責分配,制定「急救團隊角色工作分配」,舉辦「團隊擬真教育訓練模式」,及設計急救流程處置口訣「心氧氣真好記」、明確標示「急救職責及職責分配圖」,配合擬真訓練課程,加以模擬急救情境進行訓練。結果:急救處置作業正確率上升至94.6%。結論:建議可以將擬真訓練多加運用於不同情境,增加團隊默契及人員對情境熟悉度,提升作業品質與正確性。 |
英文摘要 | Background and Problem: Resuscitation skills are essential for nurses in the emergency department (ED) and encompass teamwork with other medical professionals, adherence to correct resuscitation protocols, identification of rhythm changes on the electrocardiography monitor, and proper treatment according to patient care guidelines. This quality improvement project aimed to enhance the accuracy of resuscitation procedures for ED nurses through simulation training. The project was conducted in the ED of a regional hospital in northern Taiwan. To assess the accuracy of resuscitation skills, we developed a self-administered questionnaire called the 'Observation of Emergency Nurses Operating Resuscitation Procedures.' In October 2020, we observed ED nurses during resuscitation situations and found that the accuracy of their resuscitation procedures was 62.1%. After analyzing the causes of low accuracy through interviews, we identified three key factors: (1) lack of suitable training for urgent and resuscitation situations, even for junior nurses, (2) insufficient resuscitation experience among the majority of ED nurses who were junior, and (3) unclear roles of resuscitation team members. Purpose: The project aimed to increase the accuracy rate of ED nurses' resuscitation procedures from the current level of 62.1% to over 91%. Methods: We defined the roles of members in the resuscitation team, developed the "MAIR" formula, created a "teamwork map for resuscitation", and implemented a simulation training program for nurses. Results: The accuracy rate increased significantly from 62.1% to 94.6%. Conclusion: We recommend exploring simulation training programs for different situations in the ED, which may improve the accuracy of processes and enhance implicit recognition of teamwork among medical professionals. |
