中文摘要 | 在上顎骨極度缺損但無法接受傳統補骨手術的病患來說,顴骨植牙是另一個選擇方式。許多文獻回顧皆顯示,顴骨植體成功率極高,大部分文獻統計可達97%以上。目前顴骨植牙的方式主要有三大類:傳統方式植入,上顎竇溝槽技術植入與上顎竇外側技術植入,這些技術都需要臨床的熟悉與經驗,才能避免手術中造成植入位置偏差。内視鏡輔助治療,目前在顎面外科病患來説,主要使用在於顳顎關節手術,外傷性骨折,正顎手術以及鼻竇病理性診斷爲主。對於解剖構造較爲深部的治療來說,是一項非常好的非侵入性輔助器械。本篇提出一名52歲女性病患,因長期牙周不良造成上下顎骨極度萎縮,與病患討論後,決定接受上顎以四支顴骨植體及下顎以四支植體作爲全口重建方式,在使用内視鏡輔助的方式下,可直接確定顴骨植體的位置與方向,避免傷及眼窩或是顳下窩造成併發症。根據我們所回顧文獻,爲目前全世界第二例發表,故提出此方式作爲臨床參考。 |
英文摘要 | In patients with extreme upper shin bone defect but unable to receive traditional bone repair, zygomatic implants are another option. Many literature reviews show that the success rate is very high and the literature statistics can reach more than 97%. At present, there are three main methods for dental implants: traditional techniques, sinus slot techniques, and extrasinus (exteriorized) techniques. These techniques require clinical familiarity and experience in order to avoid the implantation position deviation during surgical drilling. Endoscopic procedures are mainly used for temporomandibular joint surgery, traumatic facial bone fractures, orthognathic surgery and sinus pathological diagnosis, endosocopy for deep field of anatomy is a very useful and non-invasive tool. This article presents a 52-year- old female patient who suffered from extreme periodontal disease resulting in extreme atrophy of the upper and lower alveolar bone. After discussion with the patient, she decided to accept all-on-4-technique with 4 zygomatic implants in upper jaws and 4 implants(All-on- 4 technique) in lower jaw for full mouth graft-less reconstruction. In the use of endoscope-assisted approach, the position and direction of the zygomatic implants can be directly determined to avoid complications caused by injury to the orbital cavity or infraorbital fossa. According to our review of the previous literatures, this is the second case in the world. We proposed this method as a clinical reference. |
