中文摘要 | 探討一位26歲未婚年輕女性,因史蒂芬-強生症候群(Steven-Johnson Syndrome, SJS)致視力損傷之個案在住院期間所面臨的相關健康問題。照護期間自2015年5月15日至2015年6月3日,於眼科病房運用觀察、會談、身體評估及歐倫自我照顧缺失理論進行評估,確立個案有移位能力障礙、自我照顧能力缺失及身體心像紊亂等三項健康問題。照護過程中經由教導環境設備固定擺位、桌上物品以顏色對比引導拿取及數步數至目的地等措施,提升自我照護及移位安全。同時,以主動關懷、同理心等照顧技巧,與個案建立信任感,提供諮詢服務及介紹類似成功個案與其分享,增進個案日常照顧的能力及協助早日適應身體外觀、結構及功能上之改變。藉由此護理經驗之分享,作為日後照護類似病人之參考。 |
英文摘要 | The report described the nursing experience of caring a unmarried twenty-six years old young woman who suffered from visual impairment with Steven - Johnson syndrome during hospitalization from May 15th to June 3rd, 2015. Data were collected through clinical observation, interview, physical assessment, and Orem self-care theory in the ophthalmology ward. The health problems were identified as moving disabilities, lack of self-care skill, and body image disorders. In order to achieve security maintenance and to increase the safety of transfer, we educated the patient to have set locale of items, color contrast for on-table items, and counting of steps for destinations. Meanwhile, we used the technique of active caring and empathy to build confidence in this patient, and provided consultation and introduction of similar successful cases. Thus, the patient can deal with the physical and psychology change and feel more positive when facing the illness, and also enhance her daily care ability as soon as possible to help the patient adapt to the change of daily physical appearance, structure, and function. By sharing this nursing experience, we hope to serve as a reference for caring similar patients in the future. |
