中文摘要 | 口竇穿孔是指口腔與鼻竇不正常的相通,通常發生在上顎後牙區顎骨較薄之處,拔牙或其它各種手術之後。穿孔若未及時處理,經常演變爲急性或慢性鼻竇炎,或是口竇瘻管。現今因應植牙需求,治療口竇穿孔偏向以骨性修補爲主。本篇報告以骨鑿式上顎竇底提升術進行口竇穿孔修補,同時達到植牙部位重建的目的。手術施行於一位五十二歲的男性患者上顎右侧無牙脊區,術後八個月傷口癒合良好,電腦斷層中可見骨修復,且原先存在的竇内息肉已不復見,隨之以手術植入兩支植體,四個月後進行贋復。完成後四年的臨床追蹤與放射線報告,可見植體骨整合與功能正常。利用骨鑿式上顎竇底提升術可以有效地進行口竇穿孔修補,進而完成該區植牙的功能需求。 |
英文摘要 | Oroantral communication is a pathological communication between the oral cavity and the maxillary sinus and may occur after extraction of large molars and many other situations in the atrophied posterior maxilla. Bony closure of oroantral communication gains interest recently because of the rising demand for implant rehabilitation. Oroantral communication in a 52 year-old man was diagnosed when implant therapy was intended. Concomitant closure of the oroantral communication and implant site development was achieved by using osteotome sinus floor elevation technique with guided bone regeneration. Subsequently, two implants were placed and restored in functioning for four years. This technique could be an appreciated method to manage oroantral communication at the implant site. |
