中文摘要 | 臨床上常會遇見推著輪椅行動不便或洗腎身體虛弱之患者,前來照攝胸部X光,其可能因身體僵直、駝背和頭部無法自行抬起等因素,導致兩側肺野不對稱、肺尖及支氣管陰影被下顎遮蔽等,造成影像品質降低判讀困難。本研究目的乃藉由頭部固定輔具於頭部活動受限之輪椅使用病患行胸部X光攝影時,能提升X光影像之品質,同時亦能減少病患家屬或看護在攝影室協助攝影時,暴露於輻射劑量之機會。本研究收集自2013年10月至2014年12月,頭部無法自行抬起、身體傾斜和攣縮活動受限之輪椅使用病患,排除頸椎受傷患者,採隨機選取未使用與使用頭部固定輔具之對照組與實驗組各20人,共40人。由三位醫師依李克氏量表(Likert scale)評分法,一至五分表示非常不滿意到非常滿意之評分方式,對身體傾斜、胸腔前傾、頭部固定、頸部外展與下頜假影等,共五個項目進行影像評分。研究結果分析利用t-test之獨立樣本檢定求出二組的平均數差,其實驗組與對照組在身體傾斜、胸腔前傾、頭部固定、頸部外展與下頜假影五大項目其平均數差異比率分別為1.36、1.34、1.80、2.16 及2.97倍,顯示具有統計上顯著性差異(p﹤0.01),因此使用頭部固定輔具可協助病患下顎抬高及頭部固定,有助於改善影像品質,在臨床上具輔助診斷之意義。 |
英文摘要 | In clinical situation, we often encounter patients on wheelchairs who must undergo x-ray examinations. Due to problems such as body stiffness, hump, or difficulty in head lifting, the images of the x-ray show asymmetry of both sides of the lung field, apex and bronchial shadow covered by chin. As a result, such decreases the quality of the x-ray images and make them difficult to interpret. The purpose of this study was to exam the effects of applying head fixed aids on wheelchairs for immobile patients during chest x-ray examinations to assist patients to maintain chin lifting, head immobilization, and increase the quality of x-ray images and decrease the opportunity of extra radiation that caregiver received. Data was collected from October 2013 to December 2014. Patients were included in this study if they were on wheelchairs with difficulty keeping head lifted or midline orientation of body. Patients were excluded if they had cervical spinal injury. This study was a randomized controlled trail of forty patients. In the experimental group, twenty patients used head fixed aids while the other twenty patients were in the control group. The five items of image evaluation included body tilting, chest lordosis, head immobilization, neck extension, and chin artifact. Each was scored from 1 to 5 by three doctors according to Likert scale score. Independent t-test was used to test the differences among means for statistical significance for the experimental and control groups. The average difference ratio of the experimental group and control group are 1.36, 1.34, 1.80, 2.16 and 2.97 times for body tilting, chest lordosis, head immobilization, neck extension, and chin artifact. It demonstrated significant difference statistically. The head fixed aids group had a significantly higher mean in all of five items than control group (p<0.001). Applying the head fixed aids on wheelchair-bound and immobile patients during chest x-ray examinations can maintain head immobilization and chin lifting, and assist health providers to interpret and analysis x-ray images. |
