中文摘要 | 目的:模型室進行模具固定作業時,會因頭頸部癌病患術後頸部造口導管干擾,無法使用頭頸肩固定模具(Head and shoulder mask,HSM)。故設計一種針對造口導管的氣切套環(Tracheostomy protected casing,TPC),以協助完成頭頸肩模具製作。材料及方法:本研究分成三部分,先說明TPC製作程序,再說明加上TPC如何進行模具固定作業及觀察病患有無不適,最後將病患分成有無造口導管兩組,紀錄影像導引系統在治療前後對治療中心點的位移修正值,並分析在臨床上的意義。結果與討論:自2011年5月至2012年5月,15位有造口導管的病患,除1位因自身因素模具未完成,2位治療未完成外,其餘與另15位無造口導管的病患在製模和治療過程皆無不適現象。分析治療前後治療中心點誤差修正值,因P值均>0.05,表示TPC不會影響HSM模具擺位再現性與治療過程中的固定性;但治療前需影像導引系統修正HSM模具擺位誤差,否則應考慮增加外擴安全範圍。結論:TPC可以提供有造口導管病患安全及舒適的執行頭頸肩固定模具放射治療作業。 |
英文摘要 | Purpose: Due to the interference of the trachea tube for the head and neck patient after tracheostomy, making head and shoulder immobilization device was not achievable. Therefore, we designed a special tracheostomy protected casing (TPC) to aid the process of making head and shoulder mask (HSM). Materials and Methods: This study divided into three parts: First, describe the details process of making TPC. Second, the method we use to assess the patient comfort. Third, we divided the patient into two groups, one with TPC and the other without TPC. Later, we use IGRT images to record the setup shifts and analyze the differences between two groups and also analyze the impact on clinical decision. Results and Discussions: From May 2011 to May 2012, among fifteen patients with TPC, one of the patients was not able to complete immobilization device process and the other two cannot complete the treatment. The fifteen patients without TPC, all of them completed the immobilization process and the treatment without any discomfort. The statistical analysis on IGRT images shifts, the p-value is >0.05. The result shows the TPC do not affect the reproducibility and the immobilization of the HSM device. However, the IGRT before treatment is suggested or the safety margin should be increased. Conclusions: TPC provides patient with trachea tube comfort and safety during the process of making immobilization device for the radiation therapy. |
