中文摘要 | 霧氣治療中最常見的設備是持續流量型噴霧器(constant-output nebulizer)。除驅動氣體的流量或密度,及噴霧器內填充的溶液量之因素外、還有噴霧器內部構造設計等,均會影響噴霧器的噴霧效果,所以選用噴霧效果佳的噴霧器可使病人達到最好的療效,因此本研究目的是評估各種廠牌與型號的持續流量型小容積噴霧器之噴霧效果,並探討其與內部結構之相關性。本研究屬研究室實驗,選用10組不同廠牌和型號的噴霧器,以壓力50psig及流量8L/min的壓縮空氣驅動噴霧器,並分別測量沉降在吸氣過濾棉、配件和吐氣過濾棉上的藥量,以及噴霧器內的剩留藥量與計算噴霧時間。研究結果顯示10組持續型噴霧器的噴霧效果顯著不同,吸入藥量與噴霧器內的剩餘藥量(r=-.725;p<.001)及吐氣藥量(r=.857;p<.001)之間有明顯的相關,噴霧時間與配件上的藥物沉降量亦具有相關性(r=-.524;p<.001)。且發現各種噴霧器中的孔蓋構造所造成虹吸效果及T型管的內管高度,可能是影響噴霧器噴霧效果的原因。本研究建議製造業者再深入探討改良內部結構,並此結果可提供臨床人員選擇噴霧器時的重要參考依據。 |
英文摘要 | Constant-output nebulizer is the most commonly used device for aerosol therapy. The performance of a nebulizer is influenced by density or flow of gas source, filled volume, and its internal design. Therefore, the purpose of this study was to evaluate the performance of constant-output nebulizers with different brands and parts number and to determine the relationship between outcomes and their internal design. A bench study was conducted, and ten nebulizers marketed in Taipei were selected. Nebulizers were powered by compressed oxygen at 50 psig with flow of 8 L/min. Drug depositions on inhaled filters, accessory tubes, exhaled filters, and residual dose were calculated, and the nebulization time of each test was recorded. The results showed that the nebulizers performed differently. The inhaled mass was significant correlated with residual drug dose (r=-.725; p <.001) and exhaled drug dose (r=.857; p <.001), and the nebulization time was correlated with the drug deposition on the accessories (r=-.524;p<.001). Following analyses found that the performance of nebulizer was affected by the designs of the orifice sleeve and the internal height of a T-piece. Manufacturers further investigation on the internal design of a nebulizer for better performance is desired. The present study could potentially provide clinicians a guide of nebulizer selection. |
