中文摘要 | 高齡者在台灣慢性病醫療上是占最大族群,而高齡者服藥遵從性更往往影響其慢 性病的治療成效。台灣鄉村及城市生活形態和就醫可近性相差甚大,因此就公共衛生 角度,影響偏遠地區高齡者用藥遵從性行為的相關因素值得探討。本研究為橫斷式描 述性研究,以高雄市甲仙六龜兩區65歲以上老人為母群體,採隨機抽樣方式完成200 份問卷調查及分析。偏遠地區高齡者用藥遵從性調查結果中,在人口學變項方面,教 育程度會影響高齡者用藥遵從性,教育程度越高其用藥遵從性較佳,年齡、性別跟慢 性病罹患時間長短與用藥遵從性無統計上相關性。健康狀態及服藥特性方面,罹病種 類中睡眠障礙會降低用藥遵從性;而會服用成藥及使用健康食品的高齡者其用藥遵從 性也較低,因此需加強社區衛教及臨床衛教,讓高齡者學習更多醫藥知識,才能提高 高齡者用藥遵從性。再者,習慣於電視與電台媒體廣告購買藥物的高齡者其用藥遵從 性也較低,因此除專業人員面對面的衛教以外,更應透過媒體行銷方式來做健康衛 教,讓高齡者無形中吸收醫藥知識,進而達到提高其用藥遵從性。 |
英文摘要 | Aging is a normal, natural phenomenon, often accompanied by degradation of physiological functions and suffering from a variety of chronic diseases. Elderly accounted for the largest groups of underlying chronic diseases in Taiwan, and their medication adherence behaviors more often affect treatment effectiveness of chronic diseases. In addition, since special geographic environments in some of Taiwan's outlying islands and in remote areas, the elderly difficult to get good medical care. Therefore, understanding of the elderly medication adherence and related influencing factors in remote districts is an important public health issues. This study focused on elderly people over the age of 65 in the Jiasian and Liouguei districts, Kaohsiung city. A cross-sectional study using random samplings by way of a faceto- face interview survey (200 sets) and data further analyzed using SPSS computer software. We found that medication adherence behaviors were affected by education in the elderly, but not affected by age, gender and time of underlying chronic diseases. That is the higher education level the higher medication adherence. In addition, the poor medication adherence was found in those with insomnia, and taken the medications of over-the-counter (OTC) or nutritional supplements. We also found that those taken or purchase of OTC from TV and radio advertisement, the medication adherence were worse than the former. The study results showed that most elderly in remote regions had improper medication knowledge and their medication adherence were affected by internal and external factors such as diseases and media advertisements. Therefore, strengthen face-to-face medical education and health education by media marketing methods may be effectiveness to improve the elderly medication adherence in remote regions. |
