中文摘要 | 牙周病會造成包含牙齦、牙周韌帶、齒槽骨等牙周支持組織的破壞。患有牙周病的病患會表現嚴重的骨頭破壞、牙齒搖動和牙齒喪失,甚至合併病理性牙齒位移,影響患者的美觀及咀嚼。引導組織再生手術增進新的牙周附連,改善矯正前的狀況,讓牙齒可以移動到骨內缺損的位置並改正病理性牙齒位移。現今已有許多學者使用引導組織再生手術合併矯正治療來處理骨內缺損,然而矯正治療開始的確切時間尚未被討論出來。在本病例系列中,有嚴重骨內缺損的兩位病患以引導組織再生手術來治療,並快速以矯正治療介入。在矯正結束時和長期追蹤下,矯正及牙周的協同治療並不會影響牙周再生組織的健康,且提供了良好的臨床及放射線影像分析的結果。另一位病患在引導組織再生手術8個月後開始進行矯正治療,在術後8 個月及矯正治療結束之間的臨床探測數據並沒有太大的改變。 |
英文摘要 | Periodontal disease causes destruction of the periodontal supporting tissues including gingiva, periodontal ligament, and alveolar bone. Patients with periodontal disease often present with severe intrabony defects, tooth mobility and tooth loss; these are often accompanied by pathologic tooth migration. In such circumstances, the patient's esthetics and mastication are also compromised. Guided tissue regeneration aimed at enhancing new periodontal attachment may improve preorthodontic conditions by moving teeth into the intrabony defect and correcting pathologic tooth migration. Several authors have reported the use of combined guided tissue regeneration and orthodontic treatment when treating osseous defects. However, the optimal time to initiate orthodontic tooth movement has not been determined. In this case series, two patients with severe intrabony defects were treated with guided tissue regeneration and early orthodontic tooth movement. This combined orthodontic-periodontal approach prevented damage to the regenerated periodontal apparatus and provided excellent clinical and radiographic results at the end of orthodontic treatment and after long term follow up. The other patient was treated with orthodontic movement eight months after guided tissue regeneration. There were no significant differences in the clinical parameters between eight months after guided tissue regeneration and the end of orthodontic treatment. |
