中文摘要 | 50%肩部鈣化性旋轉肌腱病變患者有肩部疼痛的症狀。震波治療為鈣化性肌腱病變的一種非侵入性的治療方式。本研究以超音波導引震波治療棘上肌鈣化性肌腱病變,期能更精確定位震波治療部位,並評估其療效。以回溯方式收集2006至2012年間9位曾因棘上肌鈣化性肌腱病變接受超音波導引高能量震波治療的病人。比較病患治療前後的疼痛數字評價量表(numerical rating scale,NRS);並量測病患肩部X光前後觀的棘上肌肌腱鈣化區域的最大長度與寬度在治療前後的變化。結果顯示,震波治療前的平均疼痛數字評價量表為6.2±2.3,治療後則為1.0±1.2。震波治療前肩部X光鈣化平均長度為18.24±6.76 mm,寬度平均為6.48±2.63 mm。治療後鈣化平均長度則為7.16±7.30 mm,寬度則為2.36±2.34 mm。以威爾卡森符號排序檢定分析治療前後的疼痛數字評價量表及鈣化長度寬度得到的結果均有統計上的顯著差異(p<0.05)。結論:根據本研究分析,病患在接受超音波導引震波治療棘上肌鈣化性肌腱病變後,疼痛的症狀以及鈣化大小皆有改善。高能量震波提供肩部鈣化性肌腱病變一種有效而非侵入性的治療方式。 |
英文摘要 | Approximately 50% of patients with calcific tendinitis of the rotator cuff experience shoulder pain. Shock wave therapy is a non-invasive alternative treatment that is effective in pain relief, function restoration, and deposit resorption. In our study, we used ultrasonography-guided shock wave therapy for the targeted treatment of calcific supraspinatus tendinopathy.We retrospectively assessed 9 patients (8 women, 1 man; age 33-57 years) who had received echo-guided extracorporeal shock wave therapy for calcific supraspinatus tendinopathy from 2006 to 2012. Pain measured by the numerical rating scale (NRS) was compared before and after therapy. The length and width of the calcification spot on the radiographs before and after therapy were also measured.The mean NRS score was 6.2±2.3 before treatment and 1.0±1.2 after treatment. The mean calcification length was 18.24±6.76 mm before treatment and 7.16±7.30 mm after treatment. The mean calcification width was 6.48±2.63 mm before treatment and 2.36±2.34 mm after treatment. The Wilcoxon sign rank test showed that the NRS scores and length and width of calcification were all significantly better after therapy (p<0.05).The results indicate that echo-guided shock wave therapy for calcific supraspinatus tendinopathy is effective for pain relief and deposit resorption. Because of its noninvasiveness, shock wave therapy might be an option of treatment for calcific supraspinatus tendinopathy. |
