中文摘要 | 背景:近年來利用自體脂肪作為隆乳整形之使用已蔚為風潮,但感染仍為常見的併發症。常見致病菌雖為皮表菌種,但分枝桿菌之感染也不可掉以輕心。重複的清創以及正確的抗生素使用方可快速的治療分枝桿菌感染。目的及目標:及早確認為Mycobacterium Fortuitum感染可給與正確的藥物治療以及預防感染擴散。材料及方法:一健康39歲女性于美容診所接受自體脂肪隆乳,術後雙側乳房多處膿瘍產生。在廣效性抗生素使用後感染仍擴散到取脂肪之腹部及大腿。最終確認為Mycobacterium Fortuitum感染後給與正確的抗生素治療以及多次清創。結果:在多次清創手術以及抗生素治療下,Mycobacterium Fortuitum感染最後受到控制。結論:Mycobacterium Fortuitum的感染並不常見也容易被忽略。早期發現是成功治療此感染最重要的元素。延遲診斷會造成感染擴散並減低清創手術效果,抗生素使用期間延長,住院期間以及費用增加。 |
英文摘要 | Background:There has been a trend of increasing use of autologous fat breast augmentation for cosmetic purpose, it is sometimes complicated by infection. While the usual pathogen is skin bacterial flora, rare infection by Mycobacterium Fortuitum has also been reported. Multiple wound debridement with appropriate antibiotic therapy allow successful eradication of the infection.Aim and Objectives:Early identification of Mycobacterium Fortuitum for appropriate medication and prevent extensive infection.Materials and Methods:A healthy 39-year-old female received autologous breast fat augmentation at local beauty clinic. Breast multiple abscess formation with extension to fat donor site despite broad-spectrum antibiotic therapy. Upon diagnosis of Mycobacterium Fortuitum, she was treated with surgical intervention and appropriate antibiotic therapy.Results:After a series of wound debridement and antibiotic therapy, extensive infection of Mycobacterium Fortuitum was resolved.Conclusion:Mycobacterium Fortuitum infection is rare and easily neglected. Early identification is the key to successful management of such infection. Delayed diagnosis may lead to extensive infection that requires multiple debridement, prolonged antibiotic therapy, increased hospitalization time and cost. |
