中文摘要 | Time of flight MR angiography (TOF-MRA)為重要的診斷工具,但海綿竇內頸動脈容易因空氣產生磁化假影,試著利用 T2腦部影像判斷內頸動脈被空氣包覆程度,並提供 TOF-MRA參數修正參考,以減少磁化假影。使用儀器為 GE MR750 3.0T MRI,30位受檢者,26-80歲,男性 13位、女性 17位,先挑選靠近海綿竇之 T2影像並將內頸動脈分成三類,A:動脈暴露在空氣,C:動脈介於空氣和組織之間,D:動脈在腦組織內;再將病人之基底動脈 (A)和左右兩邊海綿竇內頸動脈( L and R)圈選 ROI,比較 SNR (signal to noise ratio)的差異,變數條件為 Fat saturation (Fat-Sat)有無使用。測量結果 SNR分別為, Fat-Sat組平均數和標準差為 A=402±113,C=621±110,D=747±86,沒有使用 Fat-Sat組別為, A=675±108,C=785±104, D=825±93;另外將 Basilar Artery (B)與左右兩邊內頸動脈(L and R)做比較,SNR分別為:Fat-Sat組 B=735±116,R=557±168,L=589±190;沒有使用Fat-Sat組 SNR分別為B=817±106,R=752±115,L=772±127。根據結果,A類的血管結構容易受空氣磁化假影影响,建議可事先調整 TOF-MRA條件,以防止空氣磁化假影,T2是較早完成的影像,簡單的觀察可分辨出空氣包覆海綿竇內頸動脈程度,並預估磁化假影程度以利後續影像條件調整參考。 |
英文摘要 | Time of flight MR angiography (TOF-MRA) is an important tool for internal carotid arterial (ICA) diagnosis, but susceptibility artifact has confused us because of the air interference. We used brain T2 image by adjusting MRA parameters to infer the influence level from air and fit the arterial structure more precisely. GE 750 3T was used in this study. Thirty patients were enrolled with age range 26-30. All patients have performed the MRA with and without Fat-Sat (fat saturation) techniques. The T2 images were classified into three types: A, ICA close to cavernous sinus; C, arterial between the air and tissue; D, arterial within tissue. The signal to noise ratios (SNR) with standard deviations of ICA images of left (L) and right (R) side arterials were determined and compared with basilar arterial (B). With the Fat-Sat technique, SNRs of A, C, D, B, R, and L are 402±113, 621±110, 747±86, 735±116, 557±168, and 589±190, respectively. Without the Fat-Sat technique, SNRs of A, C, D, B, R, and L are 675±108, 785±104, 825±93, 817±106, 752±115, and 772±1237, respectively. According to our study, type A was easily influenced by air; but type D would not. In terms of the basilar arterial (B) as a benchmark, we found that ICA R and L were worse than basilar A in the same way. When T2 image was executed, we suggested that the surmise of the influence from air and the adjustment of parameters would fit the arterial structure more and reduce the repeat times. |
