中文摘要 | 本文敘述一位六十歲二次腦中風後的患者因身體功能障礙,致生活功能需高度依賴家人照顧而引起憂鬱症。護理日期於2008年1月2日至2008年1月20日止,期間筆者以觀察、身體評估與會談方式收集資料,運用羅式適應模式分析問題,發現個案有身體活動功能障礙、潛在危險性傷害—自殺、跌倒,個人因應能力失調及無效性健康維護能力等健康問題。經由與個案、家屬和主要照顧者建立信任關係,共同擬定健康目標,提供支持、鼓勵與陪伴,並引介醫療資源及協助安排心理諮商和糖尿病衛教等措施,協助個案面對疾病造成的身體失能,進而接受身、心復健治療改善自我照顧能力,引導個案重建正向的自我概念。期望藉由筆者之護理經驗,提供臨床護理人員照顧類似個案之參考。 |
英文摘要 | This article describes a three weeks nursing care experience of a 60 year old man who developed depression secondary to a recurrent stroke that resulted in bodily dysfunction and a high degree of dependency on care by family members. Patient information was collected by observation, physical evaluation and interviews. Based on Roy's adaptation model, the patient's problem included (1) functional disability in physical activity, (2) risk for self-inflicted injury and fall, (3) inability to adapt, and (4) ineffective maintenance of health. Nursing interventions helped the establishment of a trusting relationship among patient, family and the primary caregiver and the formation of the patient's health goals. Other nursing activities included support, encouragement, companionship, providing resources for psychological counseling and diabetes mellitus health education. Assisting the patient to accept physical disabilities resulted in later acceptance of physical and psychological rehabilitations leading to improvement in self-care ability and a positive selfconcept. |
