中文摘要 | 正常之冠狀動脈是走在心肌層上的表面,而心肌內冠狀動脈過渡之血管,是指部份之冠狀動脈走入心肌層內,最常發生在左前降動脈。通常爲良性而無臨床意義,但部分病患會產生因運動而引發之猝死或運動引發之心絞痛。臨床上有意義的心肌內冠狀動脈過渡指的是長而深的隧道動脈,和局部心肌缺血有關。本研究旨在利用最佳的影像技術及心電圖閘門之多層切面電腦斷層,來準確的呈現心肌內冠狀動脈過渡的位置、深度及長度和相伴的冠狀動脈粥狀硬化。具不典型胸痛及有風險因子的病人,在本院接受多層切面電腦斷層掃描儀檢查,影像上符合心肌內冠狀動脈過渡即爲研究對象。檢查以16層切面電腦斷層掃描儀或64層切面電腦斷層掃描儀進行。我們發現心肌內冠狀動脈過渡最常發生在左前降動脈,尤其是中段-左前降動脈,其結果符合醫學文獻的記錄。本研究顯示心電圖閘門多層切面電腦斷層掃描儀,加上曲面合成以及橫切面影像,可以準確的準確的呈現心肌內冠狀動脈過渡的位置、深度及長度和相伴的冠狀動脈粥狀硬化;這些解剖學的細節,無法利用傳統的冠狀動脈血管攝影來評估。因此心電圖閘門之多層切面電腦斷層,在心肌內冠狀動脈過渡的診斷上,是可靠且非侵入性的。 |
英文摘要 | Myocardial bridging is a condition in which a segment of major epicardial coronary artery (most in the left anterior descending) is tunneled within myocardium. Most tunneled epicardial coronary arteries appear to be of little clinical significance, but this malformation has occasionally been associated with exercise-related sudden death and exercise-induced angina pectoris. Clinically significant myocardial bridges have a long deeply-tunneled segment and are associated with regional ischemia. The aim of this study was to use the optimal image-processing techniques to evaluate the accurate location, depth, and length of the bridged segment as well as concomitant atherosclerosis of myocardial bridging in patients examined with MDCT angiography of the coronary arteries. Patients scheduled for MDCT coronary angiography in our institution, whose image findings were compatible with myocardial bridging were enrolled in the study. MDCT was performed on 16-MDCT or 64-MDCT scanners. We found that the most common location of myocardial bridging was LAD, especially middle-LAD. The results were in agreement with studies in the literature. ECG-Gated MDCT with curved multiplanar reformation and trans-axial images is a reliable and noninvasive tool to evaluate the length, depth, precise location, and concomitant atheromatous changes of myocardial bridging. These findings are not possible to be evaluated with conventional coronary angiography. |
