中文摘要 | 「人非聖賢,孰能無錯」,以現今放射線部門各種數位儀器皆與儀器工作清單伺服器連接,可以取得正確病患資訊,但仍無法避免病人影像資料張冠李戴或方位標記誤植等人為疏忽而產生錯誤醫學影像,況且若因發現錯誤影像時間為影像已送達PACS或放射科醫師已完成該筆檢查之報告後,放射師對該筆錯誤影像所做的刪除及修正若沒有任何機制通知該報告醫師,將有可能造成影像與報告兩者間內容不一致情況發生,嚴重影響病人安全。因此我們提出一個強化的影像品管與報告流程來改善這個缺失,而作法中即包含運用改良之視覺化網頁醫學影像刪除服務來引導標的影像之確認,刪除過程中若發現報告狀態為正式,於刪除影像前須先得到該報告醫師的許可,並需根據系統所提供的影像瀏覽事件記錄由系統自動通知開單醫師及所有曾調閱過該影像之臨床醫師。本系統所有的事件記錄將完整保留,並轉換成多維度儲存體(OLAP cube:Online Analytical Processing cube)進行線上分析服務。我們希望藉此系統能讓醫學影像刪除的流程更完整,降低與避免所有與影像作業相關醫療錯誤的可能性。 |
英文摘要 | “Even Homer sometimes nods.” Nowadays, in a Department of Radiology all digital instruments are connected with a DICOM modality work-list service, providing an accurate medical history of each patient. However, certain artificial mistakes, such as mistaking one patient’s medical images and information for another, or incorrect marking of directions causes erroneous medical imaging. Moreover, patients may experience misdiagnosis and possible crisis if any error in the medical image remains undetected until after the data has been successfully delivered to PACS. Also, if a technician fails to make an instant report to the radiologist concerning any deletions or corrections to the medical image, an inaccurate medical study results. The refined imaging quality assurance paths and workflow, using a visualized web-based DICOM C-delete service, has been installed to highlight and correct the deficiency. All event logs related to the said medical incident will be entirely recorded and transformed into an Online Analytical Processing cube for further study. By using this sophisticated system, the efficiency of the workflow of the medical imaging deletion service will be improved and the possibility of medical errors reduced and eliminated. |
