中文摘要 | 背景:急診部門是醫院容易發生醫療錯誤及造成不良事件(adverse event)的地方,而異常事件的通報與分析可改善醫療錯誤。為了解急診醫療異常事件(medical incident),遂作此研究。方法:蒐集本院自九十三年九月至同年十一月三個月內,急診部門醫護人員通報的急診醫療異常事件。結果:研究期間共有24,959人次至急診,其中於急診觀察區留觀及等住院人數共9,163人次。通報醫療異常事件共有151件,其中149件確認為醫療異常事件,發生率約每1,000人6件。最易發生醫療異常事件為給藥錯誤20.8%(31件)。因異常事件而造成病人傷害12.8%(19件),但只造成輕傷害。就病患辨識發生錯誤的29件中,醫囑開立24.1%(7件)占第一位。結論:本研究中急診醫療異常事件之發生率約每千人6人次。大多數的異常事件並未造成傷害,但若有計畫性的減低急診給藥、醫囑開立、醫囑執行與檢體採送及病患辨識的異常事件,則急診病患安全會大大的改善。 |
英文摘要 | Objective: The emergency department of a hospital is one of the places where medical errors and adverse events may easily occur. The accurate reporting and detailed analysis of such incidents can help to reduce the occurrence of medical errors. This study aims to gain an understanding of medical incidents in the emergency department. Method: We collected and analyzed medical incidents reported by emergency medical staffs during a three-month period from September to November 2004. Result: During the study period, 24,959 cases were treated at the emergency department. Among them, 9,163 cases were patients who stayed overnight either for medical observation or awaiting admission. One hundred and forty nine medical incidents were identified and the occurrence rate of incidents was approximately 6 for every 1,000 cases. The medical incidents occurred in drug administration (20.8%) most frequently. Only 19 medical incidents (12.8%) caused harmful consequences to patients and they were all mildly harmful events. Twenty-nine incidents (19.5%) were attributed to patient misidentification, including, most frequently, incidents associated with medical orders (24.1%). Conclusion: In this study, medical incidents occurred in our emergency department to approximately 0.6% of medical cases. Most incidents did not cause harm. However, programs to reduce medical incidents in drug administration, medical orders, execution of a medical order, specimen collection and delivery, and patient misidentification can improve patient safety greatly. |
