中文摘要 | 本文旨在探討出生時即患有脊柱裂的學齡期兒童,手術後合併有神經性膀胱功能 障礙造成水腎及尿失禁,且因此造成病童的身體心像改變的護理問題,筆者運用十一項健康 型態加以評估及分析後確定其主要護理問題有:排尿型態改變、高危險感染、知識不足、身體 心像紊亂和母親的因應能力失調及害怕等五項。並依問題不同而提供不同的護理措施,諸如 教導病童間歇性自我導尿改善尿失禁及水腎,並藉此增進病童的自我控制感,使得這些護理 問題分別都能在病童出院前得以改善,並運用兒童發展理論協助病童克服其身體心像紊亂, 病童終能以正向的態度面對未來。藉此,筆者亦針對病童能接受並學習自我導尿的原因及自 我概念的提昇加以討論。 |
英文摘要 | The main purpose of this case study is to point out the nursing problems for a self-concept disturbance in a school-age child with a spinal bifida post-op combined neurological bladder dysfunction that resalts in hydronephrosis and incontinence. The five major nursing problems are: urinary retention, infection, insufficient knowledge, self-concept disturbance, and mother's problem including ineffective coping and fear. According to each, the appropriate nursing procedure is provided, such as a teaching program for intermittent self-catheterization. That is to help the child regain self control. Besides, to overcome the self-concept disturbance, the child development theory is used to assist the child to face the future in a positive way. Throughout the nursing process, the emphasis is put on the acceptance of self-catheterization and self-concept improvement. |
