中文摘要 | 敷料的使用對於二度燒燙傷之傷口癒合具有決定性影響。本篇報告提供以Aquacel Ag(上標 ®)治療二度燒燙傷傷口之臨床經驗。自2004年九月至十月,長庚醫院共有十二位二度燒燙傷之病患接受Aquacel Ag(上標 ®)治療;其受傷面積平均為15.9%體表面積,以Aquacel Ag(上標 ®)治療之平均面積為6.%體表面積,Aquacel Ag(上標 ®)皆在第一次清瘡手術後立即覆蓋。有十位病人之治療傷口可於覆蓋後兩週內達到95%之癒合,另外兩位於兩週後分別接受植皮手術及繼續以Aquacel Ag(上標 ®)治療。Aquacel Ag(上標 ®)平均治療時間為11.1天。我們沒有發現任何傷口感染及敷料過敏現象,除了一位病患於敷料覆蓋第二天出現血塊。對於病人覺得敷料乾燥變硬後覆蓋關節處活動僵硬,以及敷料移除時的疼痛感,本篇報告亦提供了改善的方法。我們認為Aquacel Ag(上標 ®)在二度燒燙傷之治療及傷口照顧的方便性有不錯的結果,雖然有其缺點,若妥善使用亦可作為二度燒燙傷之傷口治療的另一種選擇。 |
英文摘要 | Aquacel Ag(superscript ®), a composition of hydrofiber with 1.2% w/w silver, is a new moisture retentive topical dressing which can release silver within the dressing for up to two weeks and it is this duration that differentiates it from other sustained release silver products indicated for burn management. Between September 2004 and October 2004, 12 patients (mean age: 31.4 years) with partial thickness burns (mean: 15.9% of total body surface area) had been enrolled in this study. The study areas (mean: 6.5% of total body surface area) were covered with Aquacel Ag(superscript ®) immediately after the first surgical debridement. There were ten patients achieving 95% healing of the study areas within 2 weeks of application, the rest two patients whose wounds did not complete epithelize after removal of Aquacel Ag(superscript ®) on the 15(superscript th) day. Of these two patients, one patient received skin grafting and the other kept continuing application of Aquacel Ag and 95% of healing was achieved on the 21(superscript st) day. The mean duration of treatment with healing was 11.1 (7~21) days. There was no infection and no allergy to this dressing on the study wounds. One patient developed hematomas after application. Daily wound care was simple. Most of Aquacel Ag(superscript ®) could be left in situ until dry crust-like formation of this dressing. Patients might have movement discomfort when the treated area was over joints and also complained of pain in dressing removal. In this study, Aquacel Ag(superscript ®) demonstrated the ease of application and daily care, reducing pain associated with burns, and less expensive. In spite of some minor disadvantages, the application of Aquacel Ag(superscript ®) is safe and efficacious and can be one of the choices in management of partial thickness burns. |
