中文摘要 | 背景:精神科急重症病人因缺乏自我照顧能力、個人衛生差容易導致滋生細菌,若護理人員執行照顧前後未確實執行洗手或洗手技術不正確,可能會導致病菌傳播。2008年1月,病房發生群聚腹瀉,經院內感控小組調查發現是經由接觸傳染的諾羅病毒引起,更發現單位護理人員未確實以洗手技術減少交互感染。目的:成立專案小組旨在提升精神科急重症病房護理人員洗手成效,期能減少群聚感染事件,以維護病人健康。方法:以查檢表、要因分析圖及柏拉圖等工具蒐集資料,發現洗手設備不便、個人認知不正確、無明確精神科洗手規範、忘記及稽核制度不完善是洗手成效低原因。解決方法:配置酒精洗手設備、舉辦教育訓練、訂定精神科洗手作業標準及酒精性乾洗手作業標準、張貼提醒海報、制定稽核制度。結果:洗手執行率由64.2%提升到98.2%,洗手正確率由58.2%提升至98.7%,病房未再發生群聚感染事件。結論:本專案措施可提升精神科護理人員洗手成效,降低交互感染機率,進而促進了病人及照護者健康,建議專案措施可提供精神科臨床相關單位參考。 |
英文摘要 | For psychiatric patients, bacteria breeding may be a consequence of self care deficit, since an inaccurate hand-washing method can result in cross contamination. In the psychiatry intensive care unit (PICU), a cross infection is usually-seen to occur as nursing staff are not in compliance with the appropriate hand-washing procedures. It causes the delay of the patient's discharge from the hospital and leads to the patient's family complaints about this. The purpose of this project is to enhance the hand-washing efficacy of psychiatry PICU nursing staff, by using a check sheet, fishbone diagram and Pareto Diagram to collect and analyze data. Through the methods, we found that the reasons for the poor hand-washing efficacy were the inconvenient locations of the washing equipments, inadequate knowledge, individual forgetting, as well as indefinite standards of hand-washing and auditing. After having implemented some strategies, including supplying the equipments with alcohol-based hand rub, promoting hand-washing education, putting up posters, revising the standard of hand-washing to an explicit version, and establishing a standard for alcohol-based hand-washing and auditing, the rate of hand-washing compliance increased from 64.2% to 98.2%. In addition, the accuracy rate of hand-washing rose from 58.2% to 98.7%. Since these changes were implemented, cross-infection events have rarely occurred in PICU. The project has not only improved the hand-washing efficacy of nursing staff but also has reduced the risk of cross contamination in PICU, and has ensured both of patients' and staffs' health. |
