中文摘要 | 背景:眼框骨骨折在顏面外傷病患中常常碰到,即時重建手術可以避免手術後遺症及提早恢復眼球活動。傳統重建材料可分為自體及合成材料。多孔性乙烯聚合物植入片是一種合成材料,可以提供生物容忍性,易塑形,及足夠的效力。多孔性乙烯聚合物植入片已經在眼窩窠重建手術中佔有重要的角色。目的及目標:本報告主要評估多孔性乙烯聚合物植入片在眼窩窠重建手術中長期追蹤的結果。材料及方法:從2004年七月到2008年六月,有325位病患接收利用多孔性乙烯聚合物植入片眼窩窠重建手術,此研究針對超過兩年以上追蹤時間的病患共計148位。其中有106位男性病患及42位女性病患。平均年齡為30歲。結果分析主要包括有無眼球凹陷,複視,局限性眼球活動等症狀及徵兆及其他植入物相關的後遺症。結果:平均追蹤時間為28.5個月。術前評估有56位(37.8%)病患有複視,67位病患有(45.2%)有局限性眼球活動,手術後超過兩年追蹤發現只有2位病患有複視的情形及2位病患有局限性眼球活動。手術前有49位病患(33.1%)有眼球凹陷且兩眼突出差距大於兩厘米的情形,手術後兩年發現仍有8位病患(5.4%)有眼球凹陷的症狀。整體來說有2位病患有發生植入物感染的情形。在這次追蹤中,沒有植入物曝露,廔管形成及其他後遺症。結論:在本次長期追蹤中發現多孔性乙烯聚合植入物提供了低感染,良好的生物相容性及足夠的支撐力。以孔性乙烯聚合物在眼眶骨壁缺損重建手術中証實是有效且低併發症的人工植入物。它可對於眼眶骨壁重建時在材料的採用方面,提供一個不錯的選擇。 |
英文摘要 | Background:Porous polyethylene (Medpor) is a biocompatible synthetic material that is easily shaped and provides adequate strength in orbital reconstruction. The aim of this study is to evaluate the long-term outcomes of Medpor implants for orbital reconstruction.Aim and Objectives:Evaluate the long-term biocompatibility, infection rate and stability of Medpor in orbital reconstruction.Materials and Methods:From July 2004 to June 2008, 325 patients with orbital fractures underwent reconstruction with Medpor implants in Chang Gung Memorial Hospital. Of those, the 148 patients with a follow-up of more than 2 years were included in this study, comprising 106 male patients and 42 female patients. The average age of the patients was 30 years old. We performed an outcome analysis of the symptoms and signs of enophthalmos, diplopia, restriction of extraocular muscle movement, and implantrelated complications.Results:The average follow-up period was 28.5 months. Preoperative evaluation revealed diplopia in 56 patients (37.8% of 148 patients) and restriction of the extraocular muscle in 67 patients (45.2% of 148 patients). After a 2-year follow-up, two patients experienced diplopia and two patients experienced the restriction of the extraocular muscle. Forty-nine patients (33.1% of 148 patients) presented pre-operative enophthalmos ≥ 2mm. After the follow-up period, eight patients (16.3% of enophthalmos group) exhibited symptoms of enophthalmos. Two patients (1.3% of 148 patients) developed late implant-related infections and had their implants removed. However, no implant extrusion or fistula formation was noted.Conclusion:Porous polyethylene implants showed low infection rates and favorable biocompatibility after the long-term follow-up period. Porous polyethylene was proven to be an effective implant material and thus provides an alternative choice for orbital reconstructions. |
