中文摘要 | 氣囊漏氣測試法普遍為臨床上預估拔管後喉頭水腫的指標,研究指出氣囊漏氣量的定量大小與拔管後喘鳴有其相關性,漏氣量小之病患常有較高拔管後喘鳴的發生率,但縱觀國内外的氣囊漏氣測試之研究,對於氣囊漏氣量之技術與決定是否拔管之數值指標並無一致看法,因此本文試以文獻回顧氣囊漏氣測試法及拔管後喘鳴之結果,探討氣囊漏氣測試技術是否會影響氣囊漏氣測試以及不同數值作為預估拔管後喘鳴之成效,並進一步討論如何預防及拔管後喘鳴所給予的治療,期望能降低病患重新插管機率。以避免因重插管增加病人感染肺炎的機會或增加病患的死亡率。 |
英文摘要 | The cuff leak test is commonly used to predict the postextubation laryngeal edema that might cause the stridor in the clinics. The results of studies indicated that the cuff leak volume is significantly related to the postextubation stridor (PES). Patients who have small cuff leak volume would have higher risk of PES. However, to unique measured standard of the cuff leak test is used in the contemporary studies. On the other hand, the value of the cuff leak test to identify the PES is neither consistent. Accordingly, the major purpose of this work is to review and critique the vary techniques of the cuff leak test and their different predict outcomes of PES in these decades. Further, the preventions and treatments of PES are also detailed in this article that will help to reduce the rates of reintubation, nosocomial infection, and mortality for patients with mechanical ventilation. |
