中文摘要 | 背景:陰道壁的橫斷裂傷十分罕見。本文報告一例利用兩側下腹動脈結紮以治療 因這種疾病引起的大量產後出血。 病例報告:一16歲婦女,4小時前以真空吸引方法產下一嬰兒。陰道檢查發現在1/3陰 道的後方有一處五公分長的橫斷裂傷,直接縫合傷口多次全都失敗,改用紗布壓迫止血亦沒 成功。最後以兩側下腹部動脈結紮,才止血成功。 結論:在使用真空吸引器幫忙生產時,必須避免夾到母體的陰道壁,才可避免陰道內橫 斷裂傷。如有此等裂傷,且不能直接縫合止血,可用下腹動脈結紮,有時會有好的結果。 |
英文摘要 | Background: To evaluate hypogastric artery ligation in treatment of postpartum hemorrhage from a transverse laceration of the vagina wall in young women who desire to preserve their fertility. Case report: A 16-year-old female patient was admitted to our institution with pending shock and vaginal bleeding. She had undergone a vacuum-assisted delivery 4 hours earlier. Pelvic examination revealed a 5-cm transverse laceration in the upper part of the posterior vaginal wall. The subsequent use of primary closure and vaginal tamponade failed to stop the bleeding. Finally, she responded well after bilateral hypogastric artery ligation, without any complication in the follow-up course. Conclusion: The predisposing factor for a transverse vaginal wall laceration may be the inappropriate application of a vacuum extractor. Hypogastric artery ligation is an important alternative for the control of vaginal hemorrhage when primary suturing and packing have failed. |
