英文摘要 | Background: The aim of this study was to verify whether bronchopulmonary dysplasia (BPD) is a risk factor for asthma in very low birth weight (VLBW) infants during their early childhood. Methods: Neonates admitted to the neonatal intensive care unit (NICU) of the National Taiwan University Hospital from January 1995 through December 2001 with gestational age ≤ 32 weeks and birth body weight <1500 g were enrolled. A retrospective review of their medical charts during their hospital course was performed. Parents of the enrolled candidates were invited to answer the International Study of Asthma and Allergies in Childhood (ISAAC) core questionnaires. Atopy family history and environmental exposure risk factors were also recorded. Results: One hundred and sixty-eight preterm children completed this study. BPD was diagnosed in 31 premature children (18.5%). The mean chronological ages at assessment in the BPD and non-BPD groups were 5.6 and 4.8 years, respectively. Overall, children with BPD had significantly higher prevalence of childhood asthma than children without BPD (51.6% vs. 20.4%, p<0.001). Univariate predictors of asthma in VLBW infants included BPD, longer mechanical ventilation durations in NICU and lack of antenatal steroid usage. After multivariate logistic regression, BPD was still an independent risk factor for childhood asthma in VLBW infants (p=0.019, odds ratio (OR) =4.910). Conclusions: Children with BPD had a higher risk to develop asthma later during preschool and school ages. Antenatal maternal steroid usage appeared to help lower risk of childhood asthma among VLBW children. |
