中文摘要 | 北市某地區教學醫院於民國92年5月間,發現新生兒皮膚及耳朵感染抗藥性金黃色葡萄球菌(Methicillin-resistance Staphylococcus aureus;MRSA),由於為該單位歷年來首次發現MRSA感染個案,因此引起該院感染管制委員會的高度重視,而進行調查。材料與方法:針對5月之感染個案及所有於該期間出生之嬰兒,採回溯性研究調查,6月至8月作持續性門診病歷追蹤,調查有無相關感染個案;同期間並採取人員及環境監測:將嬰兒房所有工作人員,包括醫生、護士、醫療書記等之鼻腔進行採檢。結果:個案部分,由卡方檢定Χ2=1.0052 (df=1,P<0.05),但因感染個案太少,故再採用Yate,s corrected test =1.59375, P=0.000確認是為異常感染﹔人員部分:6月間所採檢的28位工作人員中,醫護人員金黃色葡萄球菌帶源者6人,其中2位護理人員為抗藥性金黃色葡萄球菌(MRSA),而加以隔離治療與控制﹔至於環境方面(門把、盥洗檯、嬰兒床)等並未加以培養,因為依據許多研究報告顯示,鼻腔是最常見的篩檢部位,MRSA移生也是造成病患感染的重要原因之一,因此環境方面並未立即採檢。 |
英文摘要 | It was in May 2003, that the presence of Methicillin Resistant Staphylococcus aureus (MRSA) infection of the skin and ear among newborns was detected in our district teaching hospital. This prompted the infection control committee to under-take an investigation into the outbreak. Methods and Materials: Retrospective analysis of babies that were born in May was done and the patients were all followed up the next 3 months in the outpatient department. Hospital personnel that were directly in contract with patients like doctors and nurses and other hospital workers underwent surveillance by nasal swab culture. We did not undertake any cultures of environmental factors like doorknobs, washbasins or the babys bassinets because studies had shown that the nasal area is the most common source of infection. Results showed the following: We used the Chi square method which is statistically significant at 1.0052 (df=1, P<0.05) Yate, s corrected test=1.59375, P=0.000. Out of 28 hospital personnel that were included in the study, 6 were identified as staphylococcus aureus infection carrier. 2 Out of these 6 patients were identified as having Methicillin Resistant Staphylococcus aureus (MRSA) which is identical to the ones isolated in the nursery room. The study showed the presence of an infection outbreak and the need for isolation and infection control. |
