中文摘要 | 目的:零庫存管理模式是將藥品分為穩定使用狀態與不穩定使用狀態兩大類,前 者是指用量穩定,不受季節、氣候和流行病等因素影響,後者則主要是用量波動較 大,易受疾病流行、季節、氣溫等因素影響。本研究針對使用量穩定之藥品進行分析 改善,期望能以一種新管理概念之模式做好庫存管理。 方法:本研究利用「精實醫療」,檢視「現況價值流」發掘七大浪費問題點,為 了減少藥庫庫存成本,增加儲放空間以及節省藥師因缺藥等待撥補所耗費的調劑時 間,根據現況價值流所發掘的問題點以及考慮顧客需求,制定「理想價值流」。進而 實施精實對策,運用零庫存藥品管理模式。 結果:經選定之10項藥品之庫存移轉至藥局,藥庫庫存為零。103/7/1- 103/12/31,該10項藥品每月之總庫存成本 (指藥庫+門診藥局+住院藥局之藥品成本) 與102年7月至12月同期相比,庫存成本節省了1,044萬元,平均每月節省174萬元。 減低每日撥補後藥師再向藥庫提出已撥補之藥品數量不足之需求以及藥庫辦事員 的作業時間。每日上午平均撥補所耗費工時由202分鐘減為170分鐘。 結論:雖然零庫存管理模式著實減低了醫院的庫存成本,但研究對象藥品仍受藥 商是否能如期交貨之變動因素影響。後續欲納入零庫存管理模式的藥品仍須審慎評 估,零庫存管理模式也有再修正的空間。 |
英文摘要 | Objectives: The lean healthcare implementation model separates drugs into two major categories: those in stable and unstable use states. The latter refers to those with more fluctuations in dosages, which are easily influenced by disease prevalence, seasonal changes, temperature, and other factors. This study focuses on improvements for drugs in a stable use state in hopes that a new management concept model can assist with proper inventory management. Methods: This study used the "lean healthcare" perspective to evaluate the "current value stream" to explore seven major waste problem points. To lower the inventory storage costs, increase storage capacity, and to reduce the time for pharmacists to make adjustments due to waiting for replenishment, an "ideal value stream" was established based on the problem points in the current value stream and client requirements. Results: Ten drugs were selected for stock transfer to the pharmacy, where the inventory in the drug library was zero, and a search program used to retrieve the goods issue and goods receipt transactions (billboard model). The monthly total inventory cost (drug library + outpatient pharmacy + inpatient pharmacy drug costs) of the 10 monitored drugs from July 1,2014 to December 31, 2014 were compared to the monthly total inventory costs from July to December of 2013, and the results show a reduction in inventory costs by 10.44 million NT, an average monthly savings of 1.74 million NT. Conclusions: Although the zero inventory management model did reduce hospitals' inventory costs, the drugs chosen are still affected by factors such as seasonal changes in dosage and external factors such as whether drug companies can provide stable drug supplies. The succeeding drugs in the zero inventory management model must be carefully assessed. The inventory management model still has room for improvement. |
