中文摘要 | 本研究在探討內科住院老年病患對接受導尿之感受,由研究者以自行設計之半結 構式問卷利用會談方式收集資料,研究對象是 65 歲以上的內科住院老年病患,意識清楚可 溝通,且住院期間曾有導尿經驗者。共收集 59 個個案,平均年齡為 73 歲。個案中大多數 (76.3%) 為第一次導尿。結果發現:個案之感受在身體方面,有痛、麻麻的、不舒服的感覺 ;在心理方面有情緒反應,如不好意思 (18 人 )、害怕 (4 人 )、緊張 (2 人 ) 等,但也 有部份個案 (10 人 ) 表示很期待,可鬆一口氣; 有些個案 (15.3%) 則藉轉移注意力的方 式, 如想事情、看周圍環境及心中默禱等來度過導尿過程,大多數個案 (67.8%) 對導尿時 的環境沒有特別感受, 有 12 人 (20.3%) 表示覺得環境很有安全感, 另有 6 人 (10.2%) 表示導尿時人很多,1 人 (1.7%) 覺得很冷。 對整個導尿過程之滿意度以 6 分法計,平均 為 4.71 分, 個案對導尿過程之滿意程度和由不同人員執行導尿, 在統計上有顯著差異 (P<0.05),由醫師執行導尿者滿意度較高,但其它之導尿相關經驗和滿意度高低並無統計上 之顯著差異;個案建議有:希望導尿前有解釋,希望導尿時動作輕、快一些,冷氣關小一點 ,及人不要太多。藉由以上的發現,可在日後臨床執行導尿技術時更應去注意病人的感受, 以提高護理品質。 |
英文摘要 | The purpose of this study was to investigate the perceptions of receiving urinary catheterization on hospitalized elderly patients. Data were collected through interviewing the elderly patients who had the experience of urinary catheterization during hospitalization. A semi-structured interview guide was used by the investigators to collect the information about the perceptions of receiving urinary catheterization on them. Fifty-nine elderly patients were recruited in this study, their mean age was 73. Seventy-six percent of patients were the first time being urinary catheterized. The most common physical perceptions during urinary catheterization were pain, anesthetic, and discomforts. The most common psychological perceptions were emotional responses such as feeling embarrassed, fear, and nervous. Fifteen percent of patients used displacement to cope this situation. There were ten patients expecting this procedures. Most patients (67.8%) had no specific perceptions on the environment during urinary catheterization. All patients were satisfied with the process of urinary catheterizaton. The mean score of satisfactory level was 4.7 (with a maximum score of 6). Most rated their level of satisfaction as moderate satisfactory to complete satisfactory. There was a significant difference in the level of satisfaction among patients who received the procedures from different healthy personnel. those who received catheterizations from physicians had higher level of satisfaction. Suggestions from patients included explanation before the procedure, more tender, quickly during the procedure, and turned down the air conditioning. @@ |
