中文摘要 | 先天性顱顏疾患所形成的嚴重異常咬合之處理,往往是十分地困難,同時也一直是齒顎矯正醫師積極努力的目標。先天性口腔部位的缺損如顎裂,如果過早在嬰兒時期進行手術修補,可能也會提高日後異常咬合的嚴重程度;故現今的治療方式傾向以多分科的階段性團隊治療,並配合長期追蹤評估,於適當的時機以有效率的治療方式介入,是為較理想的處理原則。Pierre Robin Sequence的嬰兒在臨床上通常呈現:1.下顎嚴重後縮2.舌頭後位3.上顎顎裂4.下顎骨角陡峭5.呼吸道阻塞。與齒顎矯正密切相關的為顎裂與下顎後縮的問題;而對於上顎顎裂的嬰兒過早進行手術修補,往往導致上顎牙弓狹窄、上下顎齒列擁擠的異常咬合。本篇病例報告即探討一位12歲Pierre-Robin Sequence男童病人接受齒顎矯正治療的過程,以及應用PAR (Peer Assessment Rating) Index來評估治療前後改善的情況。 |
英文摘要 | The orthodontic treatment with severe malocclusion due to congenital craniofacial deformity is difficult to treat. Congenital oral cavity defects such as cleft palate repaired at early infant stage will increase severity of malocclusion. The contempory philosophy of treating congenital craniofacial defects is multidisciplinary teamwork and longitudinal follows-up. Pierre Robin sequence is not a common deformity in daily orthodontic practices. It's deformities include 1.severe mandibular retrognathism 2.backward tongue position 3.cleft palate 4.high mandibular angle 5.narrowing airway. Early repaired of cleft palate usually results in narrowing dental arches and severe crowded dentitions. The case reported was a 12 year-old boy presenting Class Ⅱ, division 1 malocclusion with V-sharp arch form, upper and lower severe dental crowding, and bilateral maxillary buccal crossbite. The treatment involved extraction of teeth, application of expansion devices and fixed orthodontic appliance to correct the malocclusion as well as to improve the facial profile of the patient. The procedures of treatment were described, and the outcome assessed by PAR (Peer Assessment Rating)Index revealed the satisfactory result. |
