中文摘要 | 病因學的研究中測量誤差所導致之危險因子與罹病危險性相關性的錯誤估計一直是流行病學研究者所關心的課題。近年來,愈來愈多的流行病學研究以收取檢體來進行生物標記的檢測以增進對危險因子暴露狀態與罹病危險性探討的正確性。然而進行生物標記檢測本身也產生了測量誤差的問題。基本上,這些測量誤差可以分為兩類:一類是由於個體體內生物標記的生物性變動;另一類則是導因於實驗室進行檢測時所產生的變異。本文使用血中膽固醇濃度的測量為例來說明因個體體內生物標記產生生物性變動所引發的測量誤差及其對生物醫學研究的影響。另外,本文亦採用人類疣瘤病毒之感染與女性罹患子宮頸癌的流行病學研究為模式來探討因不同之樣本收集程序及不同之實驗室檢測技術所導致的測量誤差及其相關的研究問題。本文並提出在進行正式研究之前,先進行前驅研究來獲取測量誤差相關資料的研究架構。 |
英文摘要 | Epidemiologists have been concerned with the effects of measurement error in studies of disease etiology for a long time. In recent years, there has been increasing interest in the use of biologic markers in epidemiologic investigations. Although the application of biologic markers to epidemiology would enhance the validity of exposure and outcome measurement, the use of biomarkers provides, in turn, emerging issues of measurement errors assoc iated with assays of biological specimens. Basically, there are two broad sources of errors for biological measuremeants:1) intra-individual biological variability; and 2) errors in the act of measurement. In this paper, two scenarios of measurement errors are presented. Namely, the case of serum cholesterol measurement in cardiovascular disease epidemiology was used to illustrate errors associated with intra-individual biological variability; and the example of detection of human paillomavirus (HPV) in cervical cancer epidemiology was set to adress the issues of errors in the act of measurement. Pertinent features including the magnitude and implications of measurement errors in molecular epidemiologic studies are also highlighted. |
