中文摘要 | 目的:本研究的目的在於評估使用聽覺誘發波P300潛時所發展出來的模式,用來診斷癡呆症時的效度。 方法:第一部份先以資料庫中460位個案為對象,建立四個預測模式:P300潛時延長大於正常組的二個標準誤(大於392 msec)與對數回歸分析得到的三個預測模式。第二部份採用前瞻性資料分析,選擇癡呆症患者與憂鬱症患者為對象,來評估第一部份所得預測模式的效度。 結果:以傳統的方式(P300潛時大於392 msec)所得的效度最差,但以回歸分析所得的三個預測模式的效度相近,正確性皆可高於90%。增加N200潛時的變項,正確性也提高。採用前瞻性資料分析,來評估第一部份所得預測模式的效度後。以年齡與P300潛時的預測模式所得的效度最好,正確性、敏感度與專一度皆可高於80%,為四者中最佳的預測模式。 結論:以年齡與P300潛時為變項的預測模式是臨床上診斷癡呆症最理想的模式。 |
英文摘要 | Objective: This study was designed to test the validity of using auditory evoked potentials P300 for the diagnosis of dementia. Method: We previously elicited auditory ERPs in 460 subjects by an auditory oddball paradigm using a linked ears reference in three dementia studies. Using data from these pooled cases, we developed 4 parsimonious models for dementia diagnosis. In order to test the validity of these models, we prospectively included 152 patients with dementia and depression. Results: Logistic regression analysis to predict the probability of dementia showed that age, N200 latency, and P300 latency were the most significant contributing factors with overall more than 90% correct classification. The sensitivity and specificity were all acceptable in these 4 models based on retrospective analysis of pooled cases. Prospective testing of these 4 models showed that the age and P300 latency model fitted with reasonable validity in dementia diagnosis. Conclusion: It is concluded that of the predictivemodels tested, the age and P300 latency model is the most acceptable and is an effective tool in clinical diagnosis of dementia. |
