| 中文摘要 | 本研究的主要目的在運用經導管電腦斷層血管攝影的技術,並針對一個實際消化道出血案例,評估臨床上應用的實際效果。研究中所討論之案例為67歲女性,因有血便情形入院尋求治療,懷疑有急性消化道出血,內視鏡檢查無法發現可見的出血點。經臨床要求實施血管攝影,初次實施下腸系膜動脈血管攝影,並無發現可見疑似出血點。因此,為加強出血點偵測效果,我們將導管置放於下腸系膜動脈(SMA),實施電腦斷層血管攝影。根據檢查的執行的結果顯示,在傳統血管攝影中並無法確切得知出血點的位置,而在後續的經導管電腦斷層血管攝影中,則可清楚顯示出在空腸中段位置有疑似出血點。電腦斷層攝影,具有較佳的空間解析度,同時亦可提供病灶周邊相關的解剖位置資訊,對出血點的偵測上有相對的優勢;本案例所使用的經導管電腦斷層血管攝影,融合了此一優勢,並藉由快速且直接的注射造影劑於疑似出血的動脈血管內,增加了整體上的出血點偵測效率。 |
| 英文摘要 | The purpose of this study is to apply the catheter guided computerized tomography(CT) angiography on a patient with gastrointestinal(GI) bleeding, and to evaluate its effect on the bleeding point detection. A 67y/o female visited our hospital due to tarry stool. Acute GI bleeding was suspected, but the panendoscope from our GI department did not reveal any bleeding point. Angiography focusing on the inferior mesentery artery was followed, but no suspected bleeding point was observed. To enhance the detection of the possible bleeding points, a catheter was placed in the superior mesentery artery, and then catheter guided CT angiography was performed. A GI bleeding point at the middle potion of jejunum was clearly shown. In this case study, we found better capability of catheter guided CT angiography for bleeding points detection, compared with conventional angiography. |
