中文摘要 | 本研究目的在:(1)建立骨科護理標準,(2)建立骨科護理品質量表,(3)評值骨科護理品質。調查時間自民88年8月1日起至90年7月31日止。以某醫學中心四個骨科病房為對象,制訂骨科護理標準及護理品質量表,並將該量表應用於臨床評值,信度檢驗採觀察員間信度,效度則採專家效度及內容效度。研究結果顯示:四大目標的平均得分分別為88.24、89.61、89.20、84.34,其中以入院病人情況評估(97.46分)及入院病人環境介紹 (96.15分)兩項目最佳,但在護理計劃過程應包含家屬(75.95分)、確保病人預防感染(83.48分)、病人的護理記錄(84.51分)、評值病人對治療及護理的反應(84.52分)四個項目的得分較低。建議可透過病房在職教育,多增設個案研討相關課程,加強確保病人安全預防感染,加強同事之間意見經驗交流,提升護理記錄及護理措施之評值;善用溝通技巧,提昇病患及家屬滿意度;加強護理人員個別專業知識與技能,運用小組或團體討論出病房最適改善方案,再輔以相關改善措施。研究貢獻為建立骨科護理標準及骨科護理品質量表,將護理品質量表應用於臨床評值,提供臨床教學及服務品質改進的參考。 |
英文摘要 | The purpose of this study was to (1) establish orthopedic nursing care standards, (2) develop or tho pe dic tools of nursing care quality , (3) evaluate nursing care quality. The research started on Aug 1, 1999, and ended on Jul 31, 2001. There were 16 nurses working in 4 orthopedic units in one of the Taipei medical centers participating to develop the tools. Observers?reliability, content validity and experts' opinions were tested. The result showed that the four ob jec tive quality scores were 88.24, 89.61, 89.20, and 84.34. Assessment when patients admitted (97.46), and environment in tro duc tion to patients (96.15) were the highest scores. However, there were low scores on nursing care plan including family(75.95), infection prevention (83.48), patients?nursing records (84.51), treatment, nursing intervention and follow up(84.52). The researchers suggest arranging in-service education, dis cuss ing infection control through case reports and experts' opinion, increasing the writing quality of nursing records and improving nursing intervention ability. Com mu ni ca tion skills were used to raising pa tient's and family掇 satisfaction. Enhancing nurses' knowl edge and skill were con duct ed by team or group dis cus sion to find out the appropriate methods so as to improve the quality of nursing care. This study will help es tab lish or tho pe dic nurs ing care standards and develop eval u a tion tools. Using the tools in clinical units will not only help elevate the quality of nursing care but also increase teaching quality. |
