中文摘要 | 目的:自體脂肪常被用來修補組織的缺損。耳垂的脂肪因取得容易,且位於同樣的手術區域中,因此很適合作為修補鼓膜破損的材料。本研究提出16例以自體脂肪修補鼓膜破裂的處理經驗,以期對這種手術技術有進一步的了解。病人與方法:本研究以回溯性的方法收集16例因各種不同的原因而導致鼓膜破裂的病患,其中左耳10例,右耳6例。病人的年齡由22歲到79歲,平均53歲(12名為男性,4名為女性),平均追蹤期為21.6個月。破損的原因包括:外傷性鼓膜破裂、不活動慢性中耳炎、中耳通氣管掉落後殘存之破洞等。手術均於局部麻醉下進行,取耳垂後方之自體脂肪以修補鼓膜破洞。結果:以自體脂肪進行鼓膜成形術之16例當中有14例豉膜破損順利癒合,2例仍有殘存之破洞,成功率為87.5%。結論:耳垂後的自體脂肪可用來修補因特定原因所造成的鼓膜破損。因手術方法簡單,且併發症少,如能選擇適當的病例,以自體脂肪來修補鼓膜破損是一個不錯的手術選擇方法。 |
英文摘要 | Objective: Autograft fat is commonly used to repair tissue defects. Ear lobe fat is an easily accessible grafting material for myringoplasty. Aims of this study were to investigate the case selection, efficacy, and surgical technique of fat graft myringoplasty. Patients and Methods: Information from 16 patients (left ear: right ear 10:6, male: female 12:4 mean age 53.0 years) who underwent fat graft myringoplasty was retrospectively reviewed. The mean follow-up period was 21.6 months. Causes of ear drum perforation included trauma, inactive chronic otitis media, and residual perforation caused by previous ventilation tube insertion. Fat tissue was obtained from the ear lobe. Fat graft myringoplasties were routinely performed under local anesthesia. Results: The ear drum perforations healed smoothly in 14 of 16 patients. The success rate was 87.5%. Conclusion: Autograft ear lobe fat is a good graft material for perforated eardrums. Fat graft myringoplasty is an excellent alternative to repair the ear-drum in selected cases. |
