中文摘要 | 對腦中風病人的加護照顧,我們希望偵測已經受損的腦部是否持續受到傷害,或有否併發症產生,以利及時處理,並中止病情惡化,改善預後。另外,我們也希望能夠監測到給予中風病人的治療的療效及其併發症。因此需要依賴精確而有效的腦部監測系統,預時了解掌握病情的變化。現有的腦部監測系統大倣上可分為幾類:神經學檢查及生理監測,顱內壓監測,腦血流偵測,神經電生理,神經生化及代謝監測等。腦部監測系統的目的在於早期偵測出腦部的問題,適時處理,以避免二度傷害。目前的趨勢是以多重監測、非侵襲性、有效、便宜為主要的方向。 |
英文摘要 | The major goals of neurointensive care for stroke patient are to prevent further insult to the already injured brain, to facilitate early intervention, and therefore, to improve outcome. Accurate cerebral monitor systems are thus necessary. Moreover, therapeutic effects and complications can be evaluated by the monitor systems. The neurological examinations are the most important and basic tools for the acute stroke patients. However, sedatives and neuromuscular blockers are often used and the consciousness and motor response become difficult to evaluate. During past years, several cerebral monitor, transcranial Doppler (TCD), cerebral metabolism monitor, neuroelectrophysiology monitor, microdialysis and Neurotrend. These monitors can provide information for early detection and intervention. Reliability, simplicity, non-invasiveness and low cost are the trends of cerebral monitoring. |
