中文摘要 | 為了解麻疹、腮腺炎及德國麻疹疾病史與預防接種史和有否抗體間之關係,本研究針對2001年8月至2002年5月間,至馬偕醫院家庭醫學科,接受「出國留學體檢」的受檢者且年齡為1978年1月以後在臺灣地區出生者,共101人為研究對象。每人除詢問預防接種史,麻疹、腮腺炎、德國麻疹疾病史外,並免費抽血檢測其血清中麻疹、腮腺炎及德國麻疹IgG等三種抗體。結果發現:所有受檢者中,預防接種記錄(immunization records)有保存者只有25人(24.8%),且每位的預防接種記錄都不完整,沒有人完全知道自己三種疾病的預防接種史,部份知道者為87人(86.1%),完全不知道者為14人(13.9%)。麻疹IgG抗體陽性者有96人(95.0%),腮腺炎IgG抗體陽性者有100人(99.0%),德國麻疹IgG抗體陽性者有94人(93.1%)。知道曾經得過麻疹者有26人(27.1%);知道曾經得過腮腺炎者有18人(18.0%),知道曾經得過德國麻疹者有15人(16.0%)。101位受檢者中,有12位(11.9%)需要接種MMR疫苗。 臨床上,計劃出國留學的體檢者需要預防接種證明時,由於無法提出過去預防接種的證明文件,且礙於時間之緊迫,受檢者必須再接受疫苗注射。但由本研究之結果可知體檢者中只有11.9%需要接種MMR疫苗,因此,如何提高預防接種記錄的保存率,是值得努力的方向及目標。 |
英文摘要 | In order to assess knowledge of a personal history of natural disease or immunization for measles, mumps and rubella, we investigated the prevalence of antibodies to these viruses in students presenting to the Mackay Memorial Hospital Family Practice Clinic from August 2001 to May 2002. A total of 101 students needing a medical report for study abroad were enrolled. All were born in Taiwan after January 1978. They were asked about their immunization history and whether they had been infected with measles, mumps, or rubella. IgG antibodies to these three viruses were measured in each subject. Only 25 subjects (24.8%) had a copy of their childhood immunization records, and none of these records were complete. None of the subjects knew clearly their complete history of measles, mumps and rubella immunization or disease; 87 (86.1%) knew at least part of their history, but 14 (13.9%) had no knowledge of it at all. Antibodies to measles were present in 96 (95%), to mumps in 100 (99.0%), and to rubella in 94 (93.1%). However, only 26 (27.1%) were aware of a history of measles infection, 18 (18.0%) knew they'd had mumps, and 15 (16.0%) knew they'd had rubella. A total 12 of the 101 (11.9%) required MMR immunization. Students planning to study abroad require documentation of their immunization history. Because they often have inadequate records and are under time pressure, the usual practice is simply to give the required immunizations, whether they are needed or not. Our investigation shown, however, that only a small proportion (11.9% in this series) actually require MMR immunization. Ways of improving and preserving immunization records are badly needed. |
