英文摘要 | Planovalgus foot deformity in children with cerebral palsy is characterized by valgus of the hindfoot and a pronated posture of the midfoot with a flattened longitudinal arch. Surgical treatment focuses on subtalar joint stabilization to improve foot function. This study quantitatively evaluates the biomechanics and effects of surgery for the correction and rehabiliative treatment of planovalgus foot deformity. A Holter type, microprocessor based, portable, in-shoe, data acquisition system evaluates multiple steps of children with planovalgus foot deformity secondary to spastic cerebral palsy. Quantifiable differences in loading of the foot after surgery are identified Pre-vs. 12 months postoperative comparisons reveal the following outcomes (p ≦ 0.05): at the midfoot, the postoperative peak pressures diminish medially and increase laterally. Postoperative peak pressures are also reduced at the first metatarsal heads with an increase at the more lateral metatarsal heads. These differences are supportive of the clinical hypothesis motivating this study; that is, following foot surgery, plantar pressures are more evenly redistributed. Our findings suggest that following subtalar arthrodesis new plantar loading occurs laterally and increases total foot contact area. The clinical benefits of subtalar fusion are better stance phase stability, reduced callus formation, and improved shoe wear. Plantar pressure measurement can be quantified and supplement three-dimensional motion analysis in the evaluation of foot surgery results. |
