英文摘要 | Background: Periesophageal varices are important collaterals in patients with portal hypertension. It is difficult to evaluate them in clinical practice. The aim of this study was to compare the relative sensitivity and specificity of endoscopic ultrasonography (EUS) and computed tomography (CT) in the detection of periesophageal varices in the presence of portal hypertension. Materials and Methods: Forty patients with portal hypertension and 15 controls were enrolled. EUS was performed with an Olympus EU-M20 ultrasound fiberscope. Periesophageal varices of less than 5 mm in diameter were defined as mild varices and those of more than 5 mm were defined as severe varices. Results: None of the 15 controls without portal hypertension had periesophageal varices detected either by EUS or CT. Of 40 patients with esophageal varices, the incidence of periesophageal varices detected by EUS was 73% and by CT was 37%. The incidence of severe periesophageal varices detected by CT (22%) was similar to that detected by EUS (25%). However, while mild periesophageal varices could be detected by EUS (48%), they could less easily be seen on CT scan (15%). EUS showed the occurrence of periesophageal varices to be more frequent and more severe in patients with severe esophageal varices than in patients with mild esophageal varices (p < 0.01). Conclusion: Both EUS and CT are specific in the detection of periesophageal varices. However, EUS is more sensitive than CT and may be recommended as a standard method in the evaluation of periesophageal varices. |
