中文摘要 | 牙周病患之全口重建常因牙周組織喪失及牙齒病理性移位而增加其困難度。本報 告針對重度齒槽骨喪失合併齒列不整病例,經牙周、矯正及�G復治療後之改善情形提出討論 。兩位女性患者因牙周疾病求診,經臨床及放射線檢查後,診斷為重度牙周炎。在接受牙根 整平術,牙周手術(GTR或牙根表面處理)後,因前牙區齒列不整影響美觀,進而接受局部 矯正治療及牙釉質修整。最後利用樹脂將舌側鑄造金屬板加以固定,後牙缺牙區以牙橋重建 對於牙周組織減少伴有齒列不整之病歷,本報告合併矯正及�G復的處理方式,較藉著拔牙及 �G復以求得改善美觀之治療,具有延長齒列之完整性,使咬合力平均分佈改善美觀等優點。 綜合而言,採用合併牙周、矯正及�G復方式治療牙周組織喪失合併齒列不整之病患,可恢復 其牙周健康,改善美觀,重建咬合功能。 |
英文摘要 | It is complicated to reconstruct the edentulous area in patients with severe destruction of periodontal apparatus due to pathologic migration of the teeth and reduced periodontal support. In this cases report, we present a multidisciplinary approach to manage two periodontal-prosthesis patients. After severe periodontitis was diagnosed using radiographic and clinical examinations, the patients received basic periodontal therapy and periodontal regenerative surgery to control the periodontal inflammation as well as orthodontic treatment to align the migrated teeth and to close the spaces between the teeth to prevent food impaction. In addition, the missing teeth were replaced with fixed partial dentures and the maxillary anterior sextant was splinted using casting metal plates. The advantages of this overall treatment for these kinds of patients include not only recovery of periodontal health but also improvement of esthetics and function. |
