中文摘要 | 全民健康保險在醫療費用快速成長下,推出多項的費用抑制方法,造成對醫院經營上的重大衝擊,提高營運效率已成為醫院管理採行的重要策略。而醫院中大部分醫療資源的使用決策是由醫師掌握,且其間對資源的耗用有著很大的變異,故如何有效衡量及提昇其生產效率,實為醫院管理者重要之課題。本研究利用兩人比率效率競賽(TPREGs)與關鍵因素效率(VFE),探討某醫學中心外科系6個科別中28個醫師團隊之生產效率。分析結果,28個醫師團隊中有9個團隊為相對有效率,而由VFE之分析可將此9個團隊區分為三類,為全面性效率的團隊、多數面向為有效率的團隊以及僅有少數面向有效率的醫師團隊。管理者在規劃無效率團隊之學習典範時,除了考量個別團隊之條件與情況外,更應配合醫院之整體發展選擇適當之標竿,以達到組織效率的持續提昇與改善。 |
英文摘要 | The Bureau of National Health Insurance has proposed a number of medical cost containment measures. This development is forcing hospital administration to improve operating efficiency. Measuring and improving productive efficiency of physicians is a critical part of the cost containment strategy since physicians determine the consumption pattern of hospital resources. This study employed two-person ratio efficiency games (TPREGs) and vital factor efficiency (VFE) to analyze the productive efficiency of twenty-eight physician teams within six surgical departments in a medical center. Nine out of twenty-eight physician teams were shown to be relatively efficient in the analysis of TPREGs. The nine physician teams can be further classified into three categories in the analysis of VFE, i.e., efficient in all aspects, efficient in most of productive ratios, and efficient only in a few productive ratios. When selecting appropriate practice models for inefficient teams, in addition to the unique circumstances of individual teams, the overall planning and future development of the hospital should be taken into account in order to obtain the desired improvement in organizational efficiency. |
