中文摘要 | 為了探討玉屏風散對過敏性鼻炎及蕁麻疹施治的作用轉機,我們首先研究此方劑對於抗過敏作用及調節免疫因子之影響,並研究那一組對於細胞免疫反應正向作用最明顯,因此針對玉屏風散組成相同(黃耆:白朮:防風)劑量不同比例二組,G組=1:1:1,J組=3:1:1,及其組成黃耆(正品)、白朮、防風與晉耆(非正品),篩選15位正常人(平均年齡23.13±5.15)為研究對象,進行單核球分泌間白素-4(Interleukin-4)與中性白血球分泌組織胺(histamine)影響之研究。 結果顯示:玉屏風散於治療過敏性鼻炎及蕁麻疹,主要是抑制histamine和IL-4的釋放,進一步分析得知:防風抑制histamine和IL-4的作用比其它三味藥為佳(Dunnett's test; p<0.05),黃耆於濃度≧250μg/ml反而增加histamine的濃度(p<0.05);晉耆(佔台灣市售黃耆90%以上市場)於體外試驗整體作用最不明顯(least significant difference; p<0.05),本研究亦說明選擇真材實品對藥效相當重要;至於玉屏風散組成中,防風含量比例較高與黃耆含量比例較低組合之玉屏風散G組比J組更抑制 histamine及IL-4作用,可當為臨床上治療過敏性鼻炎及蕁麻疹之指標。 |
英文摘要 | In order to investigate the mechanisms involved in the treatment of allergic rhinitis and urticaria with Yu Ping Feng San, we first studied the effect Yu Ping Feng San had on the anti-allergy and regulatory factors of the immune system. We also attempted to determine which prescription had the reatest positive effect on cellular immunity.Thus, Yu Ping Feng San, each containing the same ingredients (Huangqi, Baizhu and Fangfeng) in different proportions and labeled G (1:1:1) and J (3:1:1), as well as Huangqi (an authentic medicinal root), Baizhu, Fangfeng and Jinqi (a non-authentic substitute for Huangqi), were used in a study on their effect on the secretion of interleuk in-4 (IL-4) from mononuclear cells and the secretion of histamine from neutrophils. Fifteen normal adults (average age, 23.13±5.15 years) were selected for participation in this study. Results indicated that Yu Ping Feng San improves conditions of allergic rhinitis and urticaria mainly by inhibiting the release of histamine and IL-4. Further analysis showed that one component, Fangfeng, played a major role in inhibiting histamine and IL-4(Dunnett's t test P<0.05); Huangqi actually increased the concentration of histamine (p<0.05 at concentration over 250μg/ml). In vitro experiments demonstrated that Jinqi (now supplying over 90% of the Huangqi market in Taiwan), produced the least significant effect (LSD; p<0.05). Therefore, this study also provides substantial evidence to support the use of authentic ingredients in traditional Chinese medicine preparations. The fact that among the two prescriptions of Yu Ping Feng San, prescription G, which contained a higher proportion of Fangfeng and a lower proportion of Huangqi that prescription J, effected greater inhibition of histamine and IL-4, can be used as a clinical guide for the effective treatment of allergic rhinitis and urticaria. |
