中文摘要 | 雖然先天性裂手畸形並不會導致功能喪失,但是如果沒有適當處理,對於病童日後的身心發展,將造成無法彌補的遺憾。先天性手裂的關閉及第一指間蹼的重建手術方式很多,皮瓣的設計方式也是玲瑯滿目,不過卻有一個共同的問題;如果不是經驗非常豐富的整形外科醫師,為了確保皮瓣的存活,我們經常無法將皮瓣與下方的結締組織做徹底分離,這或多或少都將使得日後第一指間蹼產生疤痕收縮,並使得重建之食指向橈側偏移,而影響功能。針對這個問題,我們嘗試利用顯微手術來作皮瓣的分離,將尺側指神經、血管,以及橈側指神經保留給食指,而橈側血管保留給用來製造第一指間蹼的皮瓣,將食指作近乎截肢似地分離,如此一來,可以借助顯微鏡的幫助,使得皮瓣與下方的結締組織得到完全的分離,而得到相同的手術結果。 |
英文摘要 | Although the congenital cleft hand will not cause severe loss of function, but it really causes a disability in the physical and psychological developments of the children. There are a lot of methods designed for the correction of cleft hands, but none of them are perfect. If the procedure is not performed by an experienced plastic surgeon for assuring the survival of the flaps, we often could not afford an adequate mobilization of the flap from the underlying connective tissues. This inadequate mobilization will cause the scar contracture of the first web space and radial deviation of the index finger in the future. To solve this problem, we take advantage of the microsurgery to mobilize the flap. We preserve the digital vessels and nerve of the digital side and the digital nerve of the radial side to the middle finger. The digital vessels of radial side are preserved for the flap which is going to make the new first web space. In this way, we can safely get a nearly amputated index finger and a big flap. Although this is the first case of our cleft hand reconstruction, we are very happy about the result. |
