英文摘要 | OBJECTIVE AND METHODS: Infection with Helicobacter pylori is closely associated with the development of gastritis, peptic ulcer and gastric cancer. Expression of the iceA gene was induced in H. pylori following attachment of the bacterium to the gastric epithelium. There are two gene familes, iceAl and iceA2, and the incidence of peoptic ulcer is related to the iceAl genotype. We investigated the distribution and clinical significance of the iceA genotypes in H. pyloir isolated from Taiwan. IceA genotypes from 49 H. pylori isolates determined by polymerase chain reaction. RESULTS: The iceAl allele was detected in all 49 H. pylori isolates and the iceA2 allele was detected in 29 (59%) isolates. Twenty isolates (41%) exhibited the iceAl genotype: 10 isolates (50%) from asymptomatic gastritis; 5 isolates (25%) from gastric ulcers; 4 isolates (20%) from duodenal ulcers and 1 isolate (5%) from gastric cancer. A total of 29 H. pylori isolates exhibited the iceAl iceA2 genotype: 5 (17.1%) isolates from asymptomatic gastritis; 2 (6.8%) isolate from gastric ulcers; 21 (72.4%) isolates from duodenal ulcers and 1 (3.4%) isolate from gastric cancer. Among the 25 H. pylori isolated from the duodenal ulcer patients, 21 (84%) isolates exhibited the iceA1 iceA2 genotype and 4 isolates (16%) exhibited the iceA1 genotype. The presence of the iceAl iceA2 genotype is significantly associated with the development of duodenal ulcers following H. pylori infection (p<0.05). CONCLUSIONS: In summary, all locally isolated H. pylori exhibited the iceAl allele. The iceAl iceA2 genotype in H. pilori is associated with the development of duodenal ulcers following infection with this bacteria. |
