中文摘要 | 在進行靜脈注射泌尿道攝影時,放射線科檢查室傳統士都會在注射前,將造影劑預先加溫以提高藥物的耐受性,並降低黏稠度促進注射速度。然而預先加溫究竟是否真能改善患者對經靜脈注射造影劑的耐受性,至今並無明確定論。在實際應用時,一般對所預先加溫度的多寡也很難精準掌握,因此真正的預先加溫成效並不確定。 本研究自1989年8月至1999年7月以5670名及6448名接受1VP檢查的患者作造影劑在注射前預先加溫及不預先加溫的吐較研究,以探討在注射前免除對造影劑預先加溫的可行性。 本研究發現當患者接受注射預先加溫的造影劑時,有少部分患者會在注射部位出現局部及延繽到上臂的溫熱感覺,其中也有極少數患者曾抱怨注射部位有溫燙不適的感覺;富患者接受注射不預先加溫的造影劑時,則大部分患者會使注射部位出現局部及延續到上臂的冰涼感覺,但通常只需要將手臂舉高或到注射完畢後就消失。注射前是否預先加溫,對嗎心、打噴睫、嘔吐、寒顫、眩暈、奪麻疹與搔癢等急性反應,以及全身性搔癢紅斑、眼臉/眼眶周圍浮腫等延遲性反應的發生率並無影響,此外改用較小容量注射筒,就能輕鬆完成未預先加溫造影劑的手推注射。著者主張應可取消在IVP注射之前對非離子性造影劑預先加溫的措施,這並不曾導致異常反應的發生率的提昇,但卻能簡化作業流程。 |
英文摘要 | Preheating contrast media prior to injection for intravenous pyelography is atraditional routine procedure in the department of radiology. Although it may behelpful in facilitating the rate of injection, the result of preheating the substanceis instability, therefore, we question the necessity of this routine practice. A comparative study of 5670 and 6448 patients in two groups receivingnonionic contrast media lopromide with and without preheating, respectively.prior to injection for intravenous pyelography were carried out from August 1989through July 1999. A small number of patients who received preheated contrastmedia experienced warmness locally at the injection sites and such sensation mayalso extended to the upper arm during injection. Few other patients complainedof discomfort heat sensation at the injection sites. The majority of patients whodid not receive preheated contrast media experienced coolness locally at theinjection sites and the sensation also extended to upper arm during injections. Nopatients ever complained of any discomfort in this group. There were no obviousdifferences in the incidences of general adverse reactions between the twogroups. We concluded that the preheating of contrast media prior to intravenouspyelography could be eliminated from routine procedure without additionaladverse reactions. |
