中文摘要 | 目前國內罹患中、重度神經肌肉病變損傷之肢體障礙者,大部份未經由系統化的 臨床評估及復健工程設計整合以提供符合個人需求的行動輔助系統。因此中、重度腦性麻痺 兒童常因缺乏或使用不適當的輪椅及身體支撐輔具,無法維持功能性坐姿或造成二度傷害並 引起相關併發症。本研究係針對中、重度腦性麻痺兒童,系統化的建立一套特殊輪椅及坐姿 擺位之專家系統,以提供本土化臨床評估使用、工程設計與製造具療效之個人量身訂製特殊 輪椅及坐姿擺位支撐輔具。此專家系統的開發與建立主要包括:(一)知識庫的發展,係應用 自行研發的臨床評估表、可調式坐姿擺位與支撐之評估量測系統以建立事實資料庫與規則 庫;(二)推論機的發展,係根據臨床與輔助科技指導原則和工程設計規格,比對知識庫的事 實與規則,推演出適合腦性麻痺個人所需特殊輪椅與坐姿擺位支撐輔具之臨床處方及電腦化 輔助設計;(三)推理工作編譯庫及知識蒐集模組庫的發展,係根據本研發五年來百餘位中、 重度腦性麻痺患者之諮詢、評估、輔具設計、製造、個案長期使用療效評估及改進過程中經 驗累積之建立;(四)使用者介面的發展,係針對不同專業領域使用者,提供高親和性之上述 專家系統的整合應用。本專家系統初步應用於32位重度腦性麻痺兒童,結果顯示:個別化特 殊輪椅及身體支撐輔具能有效的改善身體協調及平衡控制,提供直立之功能性坐姿,並改善 其日常生活之功能性活動。 |
英文摘要 | There is lack of assistive technology service program to provide systematical clinical evaluation and prescription of specialized seating/positioning support systems for the neuromuscular impairment in Taiwan. In addition, the fields of rehabilitation technology/assistive devices are very new with little R&D. This project is to develop an expert system in specialized seating/positioning evaluation, design and fabrication for the cerebral palsied. The goal is to provide clinical effective seating supports for minimizing abnormal muscle tone and reflex, and to improve balance control, movement coordination and functional activities for the cerebral palsied. The development and establishment of an expert seating system has included: (1) knowledge base development by applying a clinical evaluation chart and an adjustable seating and positioning evaluation system for fact, information and rule base establishment; (2) inference engine development by following clinical seating and assistive technology guidelines as well as engineering design specifications to derive seating/positioning prescription and CAD in specialized wheelchair and seating/positioning support devices for the cerebral palsied individual; (3) working memory and knowledge acquisition module development based on the results and progress of our last five-years research and development in specialized seating and positioning with more than one hundred of severe cerebral palsied children; and (4) user interface development based on the application of varying professionals with user friendly and system integration features. The development of above subsystems and clinical seating evaluation with data collection are integrated to establish the expert system. A total of 123 cerebral palsied patients with ages from 2 to 26 years old have been evaluated by using the system, 32 patients are now using the specialized wheelchairs and body support devices prescribed and developed from the system. Preliminary results are quite satisfactory. Information, knowledge, experience and results are integrated together for the content of expert system. The system is expected to provide training and education for the clinical staff and engineers as well as technology transfer in addition to provide effective assistive devices. |
