中文摘要 | 臨床前列腺增生肥大程度輕微者佔約60-80%,追蹤或服用藥物控制即可,但其中仍有部分病人會繼續增生肥大症狀加劇,而需要進一步接受手術治療。而手術後卻仍會有10-15%之病人因前列腺再度增生肥大而出現問題需再次接受切除手術。是甚麼原因導致前列腺再度增生肥大當然是一重要研究課題;另者,是否有一精確的「指標」可預期病人前列腺腺體增生肥大之增殖速率,並瞭解此「指標」是否與病患前列腺腺體的組成(腺體增生性抑或纖維肌肉增生性)、體積和前列腺特定抗原之相關性,對前列腺增生肥大之冶療及預後有極大重要性。利用特殊DNA螢光染色及快速流動細胞分析技術我們針對臨床所切除前列腺組織之細胞週期與合成速率作一前瞻性之研究分析。一年內共分析了七十五例前列腺增生肥大病人。前列腺增生肥大組織中腺體與間質細胞之比例乃以顯微鏡觀察及影像分析電腦軟體系統分析。前列腺增生肥大組織中細胞合成速率(SPF)經測定後再分別與病患年齡、腺體的組成、重量、血清前列腺特定抗原作比較分析。結果顯示前列腺增生肥大組織中細胞的合成速率與上述因子無明顯關係(相關係數,SPF vs age為O.29, SPF vs PSA為0.30, SPF vs prostate weight為-0.29, SPF vs glandular component為-0.29)。前列腺再度增生肥大(N=6)病患之細胞合成速率與初患者(N=69)並無明顯差異(P=0.059)。利用單向多變異數分析亦未見這些因子之間有任何相關性存在。此研究之結論是目前對臨床病人我們仍無法利用前列腺增生肥大組織中細胞合成速率來預測其可能之腺體的組成、重量、血清前列腺特定抗原或再生情形。 |
英文摘要 | Regrowth of prostate tissue has made secondary often unavoidable. Although the mechanisms involved in the regrowth of prostate tissue are major issue for study, it is importan for us to research for an accurate "parameter" which can predict the proliferation arte of the indi vidual prostate gland and correlate well with the hyperplasia compartment, prostate weight prostate specific antigen (PSA) level and clinical course. By using specific DNA stain dye and the rapid flow cytometric (FCM) method, the cell cycle and synthesis phase fraction (SPF) o freshly resected prostate tissue were analyzed in this study. Seventy-five cases of clinically and pathologically diagnosed prostate hyperplasia patients were enrolled in this study. The percentage of glandular-to-stroma component in entire section of tissue was measured using an image analysis software system under microscopic examination. The ratio of proliferation cells in each glandula tissue was analyzed and correlated with it's cellular components, glandular weight and serum PSA level. The preliminary results showed that there was no significant correlation between the proliferation rate of glandular cells with patient age, PSA level, prostatic weight or the glandula component ratio (correlation coefficients, τ, were 0.29 in SPF vs age, 0.30 in SPF vs PSA, 0.29 in SPF vs prostatic weight and-0.28 in SPF vs glandular component ratio individually) Also, no difference was seen in SPF between tissue non-regrowth and regrowth patients (p=0.059) One-way ANOVA revealed no significant correlation among these parameters. The measurement to cellular proliferation rate in resected prostatic tissue is not proven to be effective for prediction of glandular weight, PSA level, tissue component or regrowth possibility in clinical BPH patients. |
