中文摘要 | 背景:肥厚的下鼻甲手術方式有許多種,以雷射施行手術則有傳統手術不及的優 點。本研究乃利用釹雅各(neodymium: yttrium-aluminum-garnet, Nd: YAG)雷射,施行下 鼻甲雷射手術以解決病患鼻塞的症狀。 方法:本科自1992年10月至1994年5月,針對310名因下鼻甲肥厚造成鼻塞的病患,以釹雅各 雷射施行下鼻甲雷射手術。雷射設定能量為15瓦,每次雷射點觸時間1至2秒。平均每位病患 施行雙側手術需時15分鐘。共有164名病患可以經由問卷得到2年的追蹤結果,其中男性73名 ,女性91名,年齡由14歲到69歲平均32.4歲。 結果:在術後2年,病患主觀上獲得改善的狀況如下:鼻塞71%,流鼻水36%,打噴嚏31%,鼻 搔癢19%。結果顯示本手術對鼻塞治療有效;對流鼻水,打噴嚏,鼻搔癢的治療效果不佳。 結論:本研究顯示釹雅各雷射適用於下鼻甲肥厚造成鼻塞的治療,相較於傳統手術,並有以 下優點:出血危險性小,不需術後鼻填塞,可安排在門診手術治療。 |
英文摘要 | Background: A variety of surgical procedures are used for the reduction of hypertrophic inferior turbinates. Laser surgical procedures have some advan- tages over conventional surgery. Methods: In this study, surgery on the inferior turbinates by Nd: YAG laser was performed bilaterally on 310 patients having nasal obstruction resulting from hypertrophic inferior turbinates between October, 1992, and May, 1994. Laser spots (15 watts, 1 to 2 seconds) were applied to the medial and anteroinferior surface of the inferior turbinates. A minimum of 2 years of follow-up by questionnaires was accomplished for 164 patients (73 men and 91 women with ages betweeen 14 and 69 years, mean age 32.4 years). Results: Two years postoperatively, the overall success rate, as defined by the patient's satisfaction, was 71% for nasal obstruction, 36% for rhinorrhea, 31% for sneezing, and 19% for itching sensation. The results revealed that this surgical procedure is useful for relief of nasal obstruction, but not for sneez- ing, rhinorrhea and itching sensation. The laser procedure also involved little bleeding, required no postoperative nasal packing, and could be performed under local anesthesia in an outpatient setting. Conclusion: Nd: YAG laser could be considered as an effective and convenient tool for the reduction of hypertrophic inferior turbinates. |
