中文摘要 | 矽管脫出是施行淚管矽質管插管後常見的合併症之一,而要將矽管復位通常是 必要的,但卻不易成功,原因乃在於固定於鼻側端的環結通常已被往上拉扯到鼻淚管腔或 淚囊內。本文目的主在提出一種簡單的手術方式來解決這個問題。先取一條新的矽管重新 插入一端淚小管,再用一條6-0耐龍線,一端綁在新管,另一端則以環結綁住脫位的矽管 上,當新的矽管從鼻側端拉出後,另一端的耐能環會順勢下滑而套住舊矽管的環結,然後 整條舊矽管就可順利的拉入鼻內。 此種技術較其它方法較為簡單、安全,且成功率較高。另外要拔除矽管時卻找不到在 鼻側的矽管端時亦可應用此種方法。 |
英文摘要 | Extrusion of the silicone loop at the medial canthus is one of the most common complications following lacrimal intubation. Repositioning the tube is necessary but often difficult if the distal end of the loop with its knots had been pull up into the nasolacrimal duct or lacrimal sac. The purpose of this presentation is to introduce a simple surgical technique to resolve the problem. The authors reintubate one of the canaliculi with a piece of new silicone tube. One end of a 6-0 Nylon suture is tied to the new tube and the other end is tied into a loop around the displaced silicone tube. After pulling the piece of new silicone tube out of the nose, the loop of the Nylon suture slides and captures the original knot of the displaced silicone tube in the lacrimal sac. Then, the displaced silicone tube is pulled to its original place by the 6-0 Nylon suture. Compared with other methods, this technique is simple, safe and it provides higher successful rate. In addition, this technique may also be used to remove the silicone tube when the distal end can not be found in the nose. |
