中文摘要 | 本實驗探討牙齒根管內放置不同氫氧化鈣製劑,對根尖周圍酸鹼值之影 響。研究取因病拔除之上顎前牙,根管清創擴大及確認根尖孔之適暢後,並以立 體顯微鏡檢驗根尖孔開口大小取得樣本三十四顆。將牙齒分為三組實驗組,每組 各十顆牙齒,分別以甘油、0.9生理食鹽水及局部麻醉劑為介質調拌氫氧化鈣糊 劑,另外四顆牙齒為對照組。以螺漩渦針(lentulo spiral)將三種混合製劑分別置入 實驗組根管內,對照組則不置藥,牙冠開口處以IRM充填材封填。將各牙根浸泡 內含15c.c.生理食鹽水之塑膠瓶中,並儲於37℃密閉容器內,觀察根尖外圍液體 pH值之變化,於置藥十二小時及第一、二、三、五、七、十、十四天後,測量 根尖周圍液體pH值,經統計分析(ANOVA)及Tukey test測試不同介質三組間氫氧 根離子釋出之差異。結果顯示以氫氧化鈣糊劑置藥後pH僅在置藥初期會上升至 10,但隨時間之改變pH值則會緩慢下降;以甘油為介質之製劑可觀察到於置藥初 期有較高之pH值,可做為臨床上選擇氫氧化鈣糊劑介質之參考。 |
英文摘要 | Calcium hydroxide [Ca(OH)�� ] has been widely used in endodontic therapy. Ithas an alkaline pH of 12.5 which makes it a desirable medicament in the treatment ofinfected root canals with apical lesions. For it to act effectively as an intracanal dressing, the hydroxyl ions must be able to diffuse into periapical tissue. However, therelease of hydroxyl ions into the periapical region from the intracanal Ca(OH)�� dressingmay be influenced by different compounds when mixed with Ca(OH)�� powder. This invitro study evaluates the pH changes around root apices after dressing root canals withcalcium hydroxide mixed with three different compounds(glycerin, saline solution, ormepivacaine hydrochloride). Thirty-four recently extracted maxillary incisors of similardiameter, length, and taper were cleaned and shaped. They were randomly divided intothree groups consisting 10 teeth of each and a control group of four teeth. The teeth inthe three groups were medicated with Ca(OH)�� powder and Group A was mixed withglycerin, Group B was mixed with saline solution and Group C was mixed with mepivacaine hydrochloride, while the control group was not medicated. The coronal orifices ofall teeth were sealed with IRM. Each root was suspended in a plastic vial containing15c.c. saline solution and incubated at 37℃ - Subsequently, pH was determined by placing a pH electrode into each sample bottle at 0, 12hr, 1, 2, 3, 5, 7, 10, and 14 days. Theresults were evaluated statistically using analysis of variance (ANOVA) and Tukey testfor determining the differences among groups at various time intervals. The results ofthe three groups showed a rapid rise in pH to 10 by 12 hours, followed by a decline topH 8 over the next 3 days, then remained at pH 8 for 14 days. Group A induced favorable ion release characteristics at 12 hours, and on days 1 and 14. |
